摘 要:目的探讨酶联免疫斑点试验(ELISPOT)检测结核感染T细胞时斑点数与结核活动程度的相关性。方法回顾性分析结核感染T细胞酶联免疫斑点试验(T-SPOT.TB)检测结果阳性的73例活动性结核病患者和54例非活动性结核患者的斑点数,并研究斑点数与结核活动程度的相关性。结果与非活动性肺结核组相比,活动性肺结核组抗原A和抗原B的斑点数显著上升,差异有统计学意义(P<0.05)。活动性肺结核组中,肺内结核组与肺外结核组抗原A和抗原B斑点数差异无统计学意义(P>0.05)。以抗原A、B斑点数辅助诊断活动性肺结核,ROC曲线下面积分别为0.770和0.856。结论 ELISPOT斑点数与结核活动程度呈正相关,但与结核活动位置无关。Objective We used enzyme linked immunospot assay (ELISPOT) to study the relationship between the active de- gree of tuberculosis infection and spots of T - SPOT. TB. Methods The research objects are composed by 73 patients with ac- tive tuberculosis and 54 patients with old tuberculosis. All patients were T - SPOT. TB positive. Investigate the correlation de- gree of the number of spots and active degree of tuberculosis by a retrospective analysis of the number of spots in all patients. Results The numbers of spots of antigen A and antigen B in active tuberculosis group significantly increased compared with the inactive tuberculosis group, with the differences statistically significant( P 〈 O. 05 ). However, there is no statistical significance on the difference between active pulmonary tuberculosis group and extrapulmonary tuberculosis group ( P 〉 0.05 ). Antigen A and B spot numbers were used to assist the diagnosis of active pulmonary tuberculosis. The ROC curve was 0. 770 and 0. 856 re- spectively. Conclusion ELISPOT spots is positively associated with the degree of activity, but has nothing to do with TB activ- ity location.
