摘  要:目的探讨儿童传染性单核细胞增多症(IM)患者EB病毒抗体检测的临床价值。方法采用酶联免疫吸附法检测167例IM患儿及60例健康儿童EBV-CA-IgM、EBV-CA-IgG,采集EDTA抗凝血标本进行涂片、瑞氏染色、镜检异型淋巴细胞,并进行分析。结果在167例IM患儿中,EBV-CA-IgM抗体阳性率明显高于对照组,差异有统计学意义(P<0.05),外周血异型淋巴细胞>10%比例IM患儿也明显高于对照组,且EBV-CA-IgM和异型淋巴细胞存在相关性(χ2=6.39,P<0.05)。结论 EBV-CA-IgM是早期诊断IM的重要指标,外周血异型淋巴细胞检测是实验室辅助诊断IM的重要方法,EBV-CA-IgM与异型淋巴细胞联合检测可提高IM诊断的准确性,以弥补单独检测所致的漏诊。Objective To explore the clinical value of EB virus antibody detection in children with infectious mononucleosis (IM). Methods The expression levels of EBV - CA - IgM and EBV - CA - IgG were detected through enzyme - linked im- munosorbont method in 167 cases of children with IM and 60 cases of healthy controls ; heterotypic lymphocytes were observed and analyzed using smear of anticoagulant blood samples with EDTA, through wright staining microscopy method. Results The positive rate of EBV- CA -IgM antibody in 167 cases with IM was significantly higher than the control group, with the differ- ence statistically significant(P 〈0.05), so was the number of cases with peripheral abnormal blood lympbocytes( 〉 10% ), EBV -CA-IgM was correlated with heterotypic lymphocytes(x2 =6.39, P〈O. 05). Conclusion EBV -CA -IgM is impor- tant index for the early diagnosis of IM, and abnormal peripheral blood lymphocytes detection is one of the important assistant methods for IM. The joint detection of EBV - CA - IgM and heterotypic lymphocytes can improve the accuracy of IM diagnosis, and also make up for misdiagnosis which is usually caused by the single detection method.
