内镜下胃息肉切除术围手术期应用集束化护理的效果分析

摘　　要：目的观察集束化护理对内镜下胃息肉切除患者围手术期不良应激的改善作用。方法 86例行内镜下胃息肉切除术患者,随机分为观察组与对照组,各43例。对照组给予围手术期常规护理干预,观察组给予围手术期集束化护理干预,观察两组护理干预后不良应激改善情况[以焦虑自评量表(SAS)、抑郁自评量表(SDS)评分为评价指标],同时观察护理干预后两组术后并发症发生情况。结果观察组术后并发症发生率6.98%明显低于对照组23.26%,差异具有统计学意义(P<0.05)。两组护理前SAS、SDS评分比较差异无统计学意义(P>0.05)。护理后两组SAS、SDS评分均有所改善,但仅观察组与护理前比较差异具有统计学意义(P<0.05),且观察组护理后SAS、SDS评分(43.42±2.56)、(49.43±3.89)分均明显低于对照组(52.18±2.89)、(51.63±3.43)分(P<0.05)。结论围手术期应用集束化护理对内镜下胃息肉切除患者不良应激具有较显著的改善作用,同时可显著降低患者术后并发症发生率。Objective To observe improving effect by cluster nursing for adverse stress in perioperative period of endoscopic gastric polypectomy patients. Methods A total of 86 patients receiving endoscopic gastric polypectomy were randomly divided into observation group and control group, with 43 cases in each group. The control group received conventional nursing intervention in perioperative period, and the observation group received cluster nursing intervention in perioperative period. Observation was made on improvements of adverse reactions after nursing intervention [self-rating anxiety scale（SAS） and self-rating depression scale（SDS） scores as evaluation indexes], and postoperative complications in the two groups. Results The observation group had obviously lower incidence of postoperative complications as 6.98% than 23.26% in the control group, and their difference had statistical significance（P〈0.05）. There was no statistically significant difference of SAS and SDS scores before nursing between the two groups（P〉0.05）. After nursing, both groups had improved SAS and SDS scores, and the difference between the observation group and the control group had statistical significance（P〈0.05）. The observation group had much lower SAS and SDS scores after nursing as（43.42±2.56） and（49.43±3.89） points than（52.18±2.89） and（51.63±3.43） points in the control group（P〈0.05）. Conclusion Perioperative application of cluster nursing provides excellent improving effect for adverse stress and lowers incidence of postoperative complications in endoscopic gastric polypectomy patients.
