摘　　要：目的:探讨临床药师在冠心病合并高血压病患者药学监护中的作用。方法:临床药师参与1例冠心病合并高血压病患者的药物治疗,协助临床医师为住院患者制订个体化治疗方案,并实施全程药学监护。结果:在冠心病合并高血压病患者的治疗中,临床药师与临床医师密切合作,调整抗高血压药、抗血栓药等治疗方案,患者病情好转后出院。结论:临床药师开展以患者为中心的药学监护,有利于提高药学服务水平和服务质量,保障患者合理用药。OBJECTIVE：To explore the role of clinical pharmacists in pharmaceutical care for patient with coronary heart disease complicated with hypertension. METHODS： Clinical pharmacists participated into the drug therapy for one patient with coronary heart disease complicated with hypertension, developed individualized pharmaceutical care plans and carried out the whole course of pharmaceutical care. RESULTS ： During the treatment of coronary heart disease and hypertension, the clinical pharmacists closely cooperated with clinicians, adjusted the treatment regimen of antihypertention drugs and antithrombotic drugs, the patients discharged after getting better. CONCLUSIONS： The clinical pharmacists carry out pharmaceutical care for patient as the center can help to improve the pharmaceutical service level and service quality, ensure the rational drug use for patients.
