老年人群血栓弹力图凝血综合指数的影响因素分析

摘　　要：目的旨在对老年人群血栓弹力图的重要参数凝血综合指数(coagulation index,CI)的影响因素进行分析。方法选取2008年4月~2010年4月收集到的402例单一服用阿司匹林具有完整血栓弹力图、完整临床资料及实验室指标的老年患者,分析血栓弹力图CI值的参考值范围及其影响因素。结果 CI在老年人群中呈正态分布,参考值范围为(-3.80~3.11)。单因素相关分析显示年龄、纤维蛋白原、红细胞计数、血红蛋白、红细胞压积、血小板计数、血小板体积数与CI相关(P<0.05)。进一步行回归分析发现年龄、女性、纤维蛋白原、血小板计数与CI成正相关(P<0.01),而红细胞计数与CI成负相关(P<0.01)。结论老年人群中血栓弹力图CI值受年龄、性别、纤维蛋白原、红细胞、血小板等因素的影响。结果进一步表明,在通常情况下,年龄、女性、纤维蛋白原、血小板在血栓形成中充当正向调节的作用,而红细胞在血栓形成中充当负调控的角色。Objective To analyze the factors affecting the coagulation index （CI） in thrombelastography in elderly patients. Methods From April,2008 to April,2010,402 elderly patients with aspirin monotherapy with complete thrombelastography data , clinical data and laboratory indexes were consecutively enrolled. The reference value range of CI and the factors affecting CI were analyzed. Results CI level was normally distributed in the elderly population, and the reference value range of CI was （-3.80~3.11）. Univariate correlation analysis showed that age,fibrinogen,erythrocyte count,hemoglobin,red blood cell volume,platelet count and mean platelet volume were correlated with CI （all P≤0.05）. Regression analysis revealed that age,female gender,fibrinogen level, and platelet count were positively correlated with CI （all P 〈 0.01）,and erythrocyte count was negatively correlated with CI （P 〈0.01）. Conclusion Age,gender,fibrinogen level,and erythrocyte and platelet counts influence CI levels in thrombelastography. Moreover,the results suggested that age,female gender,and fibrinogen and platelet counts may positively regulate and erythrocytes may negatively regulate the formation of thrombosis.
