养肝益水颗粒对高血压病早期肾损害及多重心血管危险因素的影响

摘　　要：目的观察养肝益水颗粒对高血压早期肾损害病人尿微量白蛋白(MA)、β_2微球蛋白(β_2-MG)、α1微球蛋白(α1-MG)、转铁蛋白(TRF)、免疫球蛋白G(IgG)和多重心血管危险因素的影响。方法选择168例高血压病早期肾损害病人,随机分为治疗组(110例)和对照组(58例),两组均给予常规降压治疗,治疗组同时加服养肝益水颗粒。两组疗程均为12周,分别于治疗前和治疗6周、治疗12周后观察中医证候计分、临床疗效,监测血压,检测MA、IgG、α_1-MG、β_2-MG、TRF的变化和空腹血糖(FPG)、总胆固醇(TC)、低密度脂蛋白胆固醇(LDL-C)、尿酸(UA)等心血管病危险因素指标及安全性指标。结果治疗后两组中医证候计分、尿微量蛋白、血压、心血管危险因素指标均较前下降,治疗组与对照组比较,差异有统计学意义(P<0.05或P<0.01)。结论养肝益水颗粒具有改善病人症状,减少高血压早期肾损害尿微量蛋白排出,且能协同降压,降低多重心血管危险因素。Objective To observe the effects of Yanggan Yishui granule （YYG） on the levels of microalbuminuria（MA） ,β2 microglobulin（β2 - MG）,α1 -microglobulin（α1-MG）, transferrin（TRF）, immunoglobulinG（IgG） and the influence of multiple cardiovascular risk factors in patients with early renal damage induced by essential hypertension（EH）. Methods One hundred and sixty eight hypertensive patients with early renal damage were randomly divided into two groups： control group （ n = 58） treated with routine treat-ment,and treatment group （n = 110） treated with YYG in the basis of routine treatment for 12 weeks. Before treatment and 6 weeks and 12 weeks after treatment,the clinical curative effect,traditional Chinese medical syndrome score were observed. The change of blood pressure was monitored. MA, IgG,α1 - MG,β2 - MG, TRF,total cholesterol,fasting plasma glucose,low - density lipoprotein cholesterol,uric acid and other risk factors of cardiovascular disease index and safety index were observed. Results After treat-ment ,TCM syndrome score , urinary micro protein,blood pressure , cardiovascular risk factors index in two groups were lower than before treatment , there was significant difference between two groupsC P〈0.05 or P〈0.01） .Conclusion YYG has synergistic anti-hypertensive effect , can improve the symptoms,alleviate early renal damage,reduce multiple cardiovascular risk factors in hypertensive patients with early renal damage.
