精准理念在腹腔镜脾脏手术中的应用

摘　　要：目的探讨精准理念在腹腔镜脾脏手术中的应用。方法本文回顾分析20例腹腔镜脾脏手术患者的临床资料,探讨精准理念在其中的应用。结果所有患者均顺利完成手术,取得良好效果。其中行腹腔镜二级脾蒂离断脾切除术11例,腹腔镜半脾切除术3例,腹腔镜脾上极切除术2例,腹腔镜脾下极切除术4例。患者术中出血约100-550 mL,平均约200 mL,无术中大出血;患者住院天数6-9 d,平均（7.6±2.3）d;术后腹引管拔除时间3-5 d,平均（2.2±1.6）d。术后无出血、胰瘘、腹腔感染及脾热等并发症发生。结论精准理念下的腹腔镜脾脏手术是一种安全、有效的手术,有良好的应用前景。bjective To discuss the application of the precise concept in the laparoscopic splenectomy. Methods A retrospective analysis was performed on clinical data of 20 case of laparoscopic splenectomy. Results All patients had smooth course of surgery and effect desired, including 11 case of laparoscopic splenectomy with amputation of the secondary spleen pedicle, 3 case of laparoscopic semi splenectomy, 2 case of laparoscopic partial resection of the upper pole of spleen, and 4 case of laparoscopic partial resection of the lower pole of spleen. The intraoperative blood was loss 100-500 mL （mean 200 mL）, no massive hemorrhage occurred during the operation. The average hospital stay was 6-9 days, mean （7.6±2.3） days. The postoperative peritoneal drainage duration was 3-5 days, mean （2.2±1.6） days. Conclusion The laparoscopic splenectomy with precise concept that a safe and effective surgery, has a good application prospect.
