髌骨外侧关节面成形术治疗髌股外侧高压综合征的疗效观察

摘　　要：目的探讨髌骨外侧关节面成形术治疗髌股外侧高压综合征的手术技术和临床疗效。方法 2014年10月至2015年8月采用髌骨外侧关节面成形术治疗髌骨外侧高压综合征患者24例(38膝),术后指导康复训练,采用SPSS 20.0软件进行数据统计分析,以Lysholm评分系统评分,比较手术前后差异性,评价手术效果。结果 24例患者(38膝)随访6~15个月,平均9个月,膝前疼痛症状较术前明显减轻。术前Lysholm评分(52.8±4.2)分,术后(86.4±2.6)分,差异有统计学意义(P<0.05)。结论髌骨外侧关节面成形术治疗髌股外侧高压综合征不仅使髌外侧支持带得到间接松解和延长,而且通过改造髌骨形态从根本上改善了髌股关节适合度,可明显减轻髌股外侧高压综合征引起的膝前疼痛症状,临床疗效满意。Objective To investigate the surgical technique and clinical curative effect of the lateral surface of the patellaarticle plasty on excessive lateral pressure syndrome. Methods From October 2014 to August 2015, 24 cases （38 knees ） of excessive lateral pressure syndrome were treated by the lateral surface of the patella article plasty, and postoperative rehabilitation training was given. SPSS 20.0 software was used to analyze the statistical data, assessed by Lysholm scoring system, the differences were compared before and after surgery, and the effects of operation were evaluated. Results The 24 cases （38 knees） were followed up for 6 to 15 months, with an average of 9 months, anterior knee pain was significantly reduced compared with the preoperative. Lysholm score ： （52. 8 ± 4. 2） before operation, （86.4 ± 2. 6）after operation, the difference was significant（ P 〈0. 05 ）. Conclusion The lateral surface of the patella article plasty in the treatment of excessive lateral pressure syndrome, can not only loose and extend the lateral patellar retinaculum indirectly, but also improve the fitness of patell-ofemoral joint fundamentally through modifying the shape of the patella, which can significantly reduce the anterior knee pain caused by excessive lateral pressure syndrome, achieve satisfactory clinical curative effect.
