肺结核感染并哮喘患者外周血Th1/Th2细胞因子水平的变化研究

摘　　要：目的 研究肺结核感染并哮喘患者外周血Th1/Th2细胞因子水平的变化.方法 选取2015年3月至2016年3月肺结核感染并哮喘患者46例为研究组,另选40名同期体检的健康者为对照组;检测血清Th1亚群分泌的白细胞介素-2（IL-2）、细胞因子干扰素-γ（IFN-γ）和Th2亚群分泌的白细胞介素-6（IL-6）、肿瘤坏死因子-α（TNF-α）,并比较两组Th1/Th2值.结果 研究组外周血IL-2[（17.59±1.49）pg/ml]、IFN-γ[（112.40±9.60）pg/ml]显著低于对照组（P〈0.01）,IL-6[（79.93±5.42）mg/L]、TNF-α[（1.70±0.41）μg/L]显著高于对照组（P〈0.01）;研究组外周血Th1亚群比例低于对照组（P〈0.01）;对照组Th1/Th2值接近1,趋于平衡,而研究组Th1/Th2值为（0.73±0.06）显著低于对照组（P〈0.01）.结论 肺结核感染并哮喘患者外周血IL-2、IFN-γ水平显著低于健康者,IL-6、TNF-α水平显著高于健康者,Th1比例明显偏低,Th2比例明显偏高,Th1/Th2明显处于失衡状态.Objective To study the changes of peripheral blood Th1/Th2 cytokine levels in patients with pulmonary tuberculosis infection and asthma.Methods Forty-six patients with pulmonary tuberculosis infection and asthma from March 2015 to March 2016 were selected as the study group.Another 40 healthy people who received physical examination during the same period were selected as the control group.The levels of interleukin-2 （IL-2）, cytokine interferon gamma （IFN-γ） secreted by serum Th1 subsets and interleukin-6 （IL-6）, tumor necrosis factor alpha （TNF-α） secreted by Th2 subsets were detected.The Th1/Th2 value between the two groups was compared.Results The peripheral blood levels of IL-2 [（17.59±1.49） pg/ml] and IFN-γ [（112.40±960） pg/ml] in study group were significantly lower than those in control group （P〈0.01）, while the levels of IL-6 [（79.93±5.42） mg/L）] and TNF-α [（1.70±0.41） μg/L] were significantly higher than those in control group （P〈0.01）.The proportion of Th1 subsets in peripheral blood of study group was significantly lower than that in control group （P〈0.01）.The Th1/Th2 value of the control group was close to 1, and tended to balance, while the Th1/Th2 value of the study group （0.73±0.06） was significantly lower than that of the control group （P〈0.01）, which was obviously out of balance.Conclusions The levels of peripheral blood IL-2 and IFN-γ in patients with pulmonary tuberculosis infection and asthma are significantly lower than those in healthy people, while the levels of IL-6 and TNF-α are significantly higher than those in healthy subjects.The proportion of Th1 is significantly lower and of Th2 is significantly higher.Th1/Th2 is obviously in imbalance state.
