血管桥接桡动脉掌浅支皮瓣在复杂断指再植中的应用

摘　　要：目的总结血管桥接桡动脉掌浅支皮瓣在伴有皮肤缺损的断指再植修复中临床应用及效果。方法对2011年6月-2014年11月收治的6例6指伴有皮肤缺损断指，采用血管桥接桡动脉掌浅支穿支皮瓣进行修复。其中皮肤缺损合并指动脉缺损3例，合并指背静脉缺损2例．指动脉、指神经、指背静脉同时缺损1例。结果6例断指及5例皮瓣均完全成活，1例皮瓣出现边缘部分坏死，经外科换药愈合．无血管危象病例。术后随访2-16个月，平均6个月，再植指体无萎缩，皮瓣质地良好，再植手指及皮瓣外形满意，手指功能恢复满意。结论应用血管桥接桡动脉掌浅支皮瓣修复伴皮肤缺损断指可获得良好的临床效果。Objective To summarize the clinical effect of radial artery vascular in bridging the superficial palmar branch flap with skin defect in replantation repair. Methods From June 2011 to November 2014, 6 cases （6 severed fingers） with skin defect using the radial artery vascular bridging superficial palmar branch perforator flap to repair, including the skin defect combined with artery defect in 3 cases, combined dorsal vein defect in 2 cases, and artery, nerve and dorsal vein simultaneously defect in 1 case. Results 6 cases severed fingers and 5 cases flaps survived, 1 flap edge necrosis occurred, healed by surgical dressing, no vascular crisis cases. For following up for 2 to 16 months, an average of 6 months, no shrinking body of replanted fingers, the flaps had good texture, the replanted fingers and flaps obtained satisfactory appearance, finger function recovered satisfactorily. Conclusion It can achieve good effect that using vascular bridging superficial palmar branch perforator flap to repair severed finger with skin defect.
