超声检测胎儿脐动脉血流对预测帆状胎盘孕妇预后的价值分析

摘　　要：目的:分析超声检测胎儿脐动脉血流对预测帆状胎盘孕妇预后的价值。方法:选取2015年1月至2016年1月我院就诊的单胎帆状胎盘孕妇92例,按妊娠结局情况分为妊娠结局良好组(n=48例)和妊娠结局不良组(n=44例),选择同期健康孕妇40例作为对照组,应用脉冲多普勒超声测定脐动脉血流S/D比值(收缩期峰值流速/舒张末期流速)及阻力指数。结果:妊娠结局不良组S/D比值、阻力指数均较妊娠结局良好组、健康孕妇组高(P<0.05);随着胎儿脐动脉血流S/D比值增加,胎儿分娩孕周提前,出生体质量下降,且胎儿不良结局率提高。结论:产前超声测定脐动脉血流S/D比值时,对预测帆状胎盘孕妇预后有一定的临床价值。Purpose： to analyze the ultrasonic detection of fetal umbilical artery blood flow in predicting the prognostic value of velamentous placenta in pregnant women. Methods： from January 2015 to January 2016 in our hospital for treatment of single Taifan placenta 92 eases of pregnant women, according to the outcome of pregnancy were divided into good outcome of pregnancy group （ n = 48 cases） and adverse pregnancy outcome group （ n = 44 cases ）. Select the same period 40 cases of healthy pregnant women as control group, umbilical blood flow ratio of S/D Doppler ultrasound pulse （ peak systolic velocity and diastolic velocity） and the resistance index. Results： the adverse pregnancy outcome group S/D ratio, resistance index was a good outcome of pregnancy group That healthy pregnant women group （ P 〈 0. 05 ） ; with the increase of fetal umbilical artery S/D ratio, fetal gestational weeks in advance, birth weight decreased, and the adverse outcome rate. Conclusion： the determination of fetal umbilical artery blood flow S/D ratio of prenatal ultrasound, the prediction has a certain clinical value of velamentous placenta maternal prognosis.
