摘  要:目的探讨同源盒基因A13(homeobox A13,HOXA13)蛋白的表达水平与激素耐药型肾病综合征的关系。方法选取2014年4月至2016年10月沈阳市第四人民医院收治的15例原发性肾病综合征患儿为研究对象,按照是否对糖皮质激素耐药将其分为激素敏感组(9例)和激素耐药组(6例),采用半定量评分法评价肾小管间质病理损害程度,免疫组织化学法检测肾小管上皮HOXA13蛋白的表达水平。结果激素耐药组患儿肾小管间质半定量评分显著高于激素敏感组(P<0.05),激素耐药组患儿HOXA13蛋白表达水平明显低于激素敏感组(P<0.05),且激素耐药组患儿HOXA13蛋白表达水平与肾小管间质病理损害评分呈负相关(r=-0.892,P<0.05)。结论原发性肾病综合征患儿肾小管上皮细胞HOXA13蛋白表达水平下降可能与糖皮质激素耐药有关。Objective To investigate the relationship between the expression level of homeobox A13 (HOXA 13) protein and steroid resistant nephrotic syndrome. Method 15 cases of primary nephrotic syndrome in Shenyang Fourth People's Hospital from April 2014 to October 2016 were selected as the research objects, they were divided into steroid-sensitive group (n = 9) and steroid-resistant group (n = 6), semiquantitative scoring method was used to evaluate the degree of tubulointerstitial lesions, the expression of HOXA13 protein in renal tubular epithelial cells was detected by immunohistochemistry. Result In steroid-resistant group, the semi quantitative score was significantly higher than that in steroid-sensitive group (P 〈 0.05); the expression level of HOXA13 protein in steroid-resistant group was significantly lower than that in steroid-sensitive group (P 〈 0.05). The expression level of HOXA13 protein was negatively correlated with the pathological score of tubulointerstitial lesions in children with steroid resistance (r = -0.892, P 〈 0.05). Conclusion The decreasing expression of HOXA13 proteinc in renal tubular epithelial cells in children with primary nephrotic syndrome may be related to glucocorticoid resistance.
