补气活血通络法治疗椎-基底动脉供血不足临床观察

摘　　要：目的:观察补气活血通络法治疗椎-基底动脉供血不足的临床疗效。方法:纳入70例椎-基底动脉供血不足患者,按照随机双盲法分为治疗组与对照组各35例,治疗组给予中医补气活血通络法治疗,对照组给予尼莫地平片治疗,2组均以7天为1疗程,连续治疗2~3疗程。检测2组患者治疗前后右椎动脉(RVA)、左椎动脉(LVA)、基底动脉(BA)的血流速度与血液流变学指标,统计2组患者治疗后的临床疗效。结果:治疗后,治疗组与对照组总有效率分别为94.29%、68.57%,2组比较,差异有统计学意义(P<0.05)。2组RVA、LVA、BA的血流速度与血浆黏度、全血黏度、纤维蛋白原、血细胞比容等血液流变学指标均较治疗前有一定程度的改善,治疗组各指标的改善情况均优于对照组,差异均有统计学意义(P<0.05)。结论:采用中医补气活血通络法治疗椎-基底动脉供血不足的疗效优于单纯西药治疗,可有效改善患者RVA、LVA、BA的血流速度及血液流变学指标,有效缓解症状。Objective：To observe the clinical effect of qi-supplementing,blood-activating and collateral-dredging therapy in treating vertebro-basilar insufficiency. Methods ：Selected 70 cases of patients with vertebro-basilar insufficiency, and divided them into the treatment group and the control group randomly using random double-blind method,35 cases in each group. The treatment group was treated with qi-supplementing,blood-activating and collateral-dredging therapy of Chinese medicine, and the control group was treated with Nimodipine tablets. The treatment course was 14 to 21 days. Detected blood flew velocity and hemorheology indexes of right vertebral artery（RVA）,left vertebral artery（LVA） and basilar artery（BA） in patients in both groups by transcranial Doppler ultrasound before and after treatment. Collected the clinical effect in both groups after treatment. Results ：After treatment,the total effective rate was 94.29% in the treatment group and 68.57% in the control group,the difference being significant（P〈0.05）. Plasma viscosity,whole blood viscosity,fibrinogen,hematocrit and other hemorheology indexes of RVA,LVA and BA in both groups were improved to some extent when compared with those before treatment. Improvements of all indexes in the treatment group were better than those in the control group（P〈0.05）. Conclusion：The therapy of qi-supplementing, blood-activating and collateral-dredging therapy for vertebro-basilar insufficiency has better curative effect, comparing with that of the therapy of pure western medicine. It can effectively improve blood flew velocity and hemorheology indexes of RVA,LVA and BA to significantly relieve symptoms.
