摘  要:目的研究原发性早泄病人阴茎头敏感度与阴茎背神经局部解剖学的关系。方法收集2015年1月至12月在我院泌外二科行阴茎背神经选择性切断术的47例病人,术前阴茎生物震感测定仪检测阴茎头敏感度,对测定结果和手术中游离出阴茎背神经结果进行统计学分析。结果 47例病人手术中发现阴茎背神经数目为6支1例,8支7例,9支7例,10支6例,11支12例,12支5例,13支6例和14支3例。术前病人满意率(32±6)%,性伴侣满意度(25±9)%。术后显效42例,占89.36%,好转4例,占8.51%;无变化者1例,占2.13%。术后病人满意率(85±9)%,性伙伴满意率(82±11)%。阴茎头敏感度最大3.47V,最小1.59V,平均2.63V。术前阴茎头敏感度、阴茎体敏感度均与术中阴茎背神经数目成负相关性,阴茎头与阴茎体敏感度无明显相关性。结论 (1)阴茎生物震感阈值测定法是一种无创、可定量和重复性好的阴茎背神经兴奋性筛选方法;(2)本研究中病人阴茎头敏感度平均值为2.63V,阴茎体敏感度平均值为3.06V;(3)本研究结果提示阴茎头敏感度越高的部位阴茎背神经分布越多,越稠密。Objective To study the relationship between the glans sensitivity and dorsal penile nerve regional anatomy in the patients with primary premature ejaculation. Methods From January 2015 to December 2015, a total of 47 patients, who were enrolled in the Second Department of Urology in our hospital, underwent the selective resection of dorsal penile nerve. Their glans sensitivity was measured by the Penile Biothesiometer before the operation. The data for glans sensitivity and the nerves separated in the operation were statistically analyzed. Results The amount of nerves for the patients in the operation was 6 nerves (1 case), 8 (7 cases), 9 (7 cases), 10 (6 cases), 11 (12 cases), 12(5 cases), 13(6 cases) and 14 (3 cases), respectively. The pre-operative satisfaction rate of the patients was 324-6%, the satisfaction rate of sexual partner was 25±9%. Of all patients, 42 were cured after operation (89.36%), 4 cases improved (8.51%), one case no change (2.12%).The post-operative satisfaction rate of the patients was 85±9%, and the satisfaction rate of partner was 82±1%. The maximum glans sensitivity was 3.47V, while the minimum was 1.59V, the mean value was 2.63V. The pre-operative glans sensitivity and penis sensitivity were negatively correlated with the amount of dorsal nerves, the glans sensitivity was not correlated with penis sensitivity. Conclusion (1) The Penile Biothesiometer is a noninvasive, quantitative and reproducible screening technique to measure the excitability of penile dorsal nerve. (2) In our research, the mean value of glans sensitivity was 2.63V, the penis sensitivity was 3.06V. (3) This study indicates that higher glans sensitivity may be dependent of amount of dorsal penile nerves.
