新生儿胸腹手术不同保暖方式应用效果的回顾性分析

摘　　要：目的:观察新生儿胸腹手术中保暖方式的选择对患儿术后恢复效果。方法:随机抽取2014年1月-2016年3月在本院手术室行胸腹手术的新生儿100例作为研究对象,根据保暖方式的不同分为A、B、C和D组,每组各25例。观察四组患儿手术前后体温变化、术中出血量、手术前后血氧饱和度及心率、手术时间、麻醉恢复期间寒战发生情况并使用统计学方法进行比较。结果:(1)A、B、C三组手术开始60 min与手术结束后的平均体温高于D组(P<0.05),而三组之间比较差异无统计学意义(P>0.05);(2)四组患儿术中出血、手术前后血氧饱和度、手术时间比较差异均无统计学意义(P>0.05);(3)D组术后心率低于A、C组(P<0.05),其他组间比较差异均无统计学意义(P>0.05);(4)A组麻醉恢复期寒战发生率低于D组(P<0.05),其他组间比较差异均无统计学意义(P>0.05)。结论:新生儿术中应用加温毯与薄膜保温加四肢包裹套两种保暖方式均对新生儿术后体温恢复、减少不适有理想的效果,其中加温毯且四肢使用包裹套效果最为显著,但加温毯造价较高,可以根据患儿病情及经济条件加以选择。Objective：To observe the recovery effect of the choiced with thermal ways after operative in newborn chest and abdomen surgery.Method：A total of 100 newborn with chest neonatal surgery were randomly extracted as the research objects from January 2014 to March 2016 in our hospital operating room,they were divided into four groups according to its warm ways,25 cases in each group.Ttemperature changes before and after operation,intraoperative blood loss,blood oxygen saturation and heart rate before and after operation,operation time and chills during anesthesia recovery of four groups were observed and compared with statistical methods.Result：（1）Average temperature of the A,B and C groups in the operation began at 60 min and after the end of surgery were higher than that of the D group（P〈0.05）,there were no statistical significance in other comparisons（P〉0.05）.（2）There were no statistical significance in intraoperative bleeding,blood oxygen saturation or operation time of four groups before and after operation（P〉0.05）.（3）The heart rate of the D group after operative was lower than that of the A and C group（P〈0.05）,there were no statistical significance in other comparisons（P〉0.05）.（4）The chills incidence of anesthesia recovery of the A group was lower than that of the D group（P〈0.05）,there were no statistical significance in other comparisons（P〉0.05）.Conclusion：It has a ideal effect on recovery of body temperature and reduction of discomfort to use heating blanket or thermal insulation film plus with limbs covers during operation,and the heating blanket and limbs covers have the most significant effect,but the cost of these are higher,it depends on patients and economic conditions.
