疼痛综合护理管理对胃癌手术患者术后生活行为方式的影响

摘　　要：目的:分析疼痛综合护理管理对胃癌手术患者术后生活行为方式的影响,以期寻找改善胃癌手术患者术后生活行为方式的有效护理措施,降低术后复发率,提高临床疗效。方法:选择2015年1~11月我院行胃癌根治术患者128例为研究对象,随机将其等分为试验组和对照组,对照组术后采用胃癌手术临床护理路径对其进行护理,试验组患者则采用疼痛综合护理管理对患者进行术后护理。比较两组患者术后3个月生活行为方式(抽烟、饮酒、体育锻炼、饮食规律性、食盐量、焦虑抑郁评分、睡眠质量的变化);随访1年,统计比较两组患者的生存情况、肿瘤患者生活质量评分(QOL)。结果:两组患者临床基线资料(性别、年龄、胃癌分期、Karnofsky功能状态评分、手术方式、术前肿瘤患者生活质量评分)及术前生活行为方式各项指标调查比较差异均无统计学意义(P>0.05);术后3个月,两组患者的生活行为方式较术前有明显改善,嗜烟、嗜酒、缺少体育锻炼、饮食不规律、高盐、焦虑、抑郁、睡眠障碍等情况均较术前有明显改善(P<0.05),但试验组患者上述指标明显低于对照组(P<0.05);试验组患者肿瘤患者生活质量评分高于对照组,差异有统计学意义(P<0.05)。结论:疼痛综合护理管理可有效改善胃癌手术患者的生活行为方式,建立更有利于病情康复的行为生活方式,对提升胃癌手术治疗患者生命质量具有较高的临床价值。Objective ： To analyze the influence of comprehensive pain nursing management on post - operation life behavior style of patients receiving gastric cancer operation, so as to find out the effective nursing measures to improve the post - operation life behavior style of patients receiving gastric cancer opera- tion, reduce the post -operation recurrence rate and improve the clinical curative effect. Methods： Selected 128 patients receiving radical gastreetomy for cancer in our hospital from January to November 2015, and they were equally divided into test group and control group, gastric cancer operation clinical nursing route was used for the nursing of patients in the control group after operation, and comprehensive pain nursing management was used for nursing of patients in the test group after intervention. Patients in the two groups were compared in the life behavior style 3 months after operation （ smoking, alcohol drinking, physical exercise, diet regularity, salt intake, score of anxiety and depression and change of sleep quality） ; 1 year＇ s follow - up visit was paid, and a statistical comparison was made on the scores of survival condition and life quality of tumor patients of the two groups. Results：Through comparison of the clinical baseline data （ gender, age, gastric cancer staging, Karnofsky functional state score, operation method, llfe quality score of tumor patients be- fore operation ） and investigation on various indicators of life behavior style before operation of patients in the two groups, the difference was not statistically significant （P 〉 0.05 ） ; 3 months after operation, the life behavior style of patients in the two groups was improved significantly compared with that before operation, smoking, alcohol abuse, lack of physical experience, irregular diet, high salt intake, anxiety, depression, sleep disorder and other situations were significantly improved compared with those before operation （ P 〈 0.05 ） , but the above indicators of patients in the test g
