白虎汤加减联合胸腺肽α_1治疗老年重症肺炎患者的效果

摘　　要：目的探讨自拟升降白虎汤联合胸腺肽α_1治疗老年重症肺炎患者的临床疗效。方法选取64例老年重症肺炎患者,采用随机平行对照法将其分观察组和对照组各32例。入院后均给予常规抗感染、止咳平喘、营养支持等常规处理,对照组给予胸腺肽α_1皮下注射治疗,观察组在对照组基础上加用自拟升降白虎汤鼻饲治疗。持续治疗2周后观察2组患者细胞免疫功能指标(CD_3^+、CD_4^+、CD_8^+、CD_4^+/CD_8^+)和血清炎症指标[白细胞计数(WBC)、C-反应蛋白(CRP)、白介素-6(IL-6)]变化,比较2组疗效、不良反应发生情况、住院时间、医疗费用。结果 2组治疗后CD_3^+、CD_4^+、CD_8^+、CD_4^+/CD_8^+、WBC、CRP、IL-6较治疗前均显著改善(P<0.05);观察组治疗后CD_3^+、CD_4^+较对照组显著升高,WBC、CRP、IL-6较对照组显著降低(P<0.05);2组均未出现明显不良反应,观察组治疗显效率65.62%,显著高于对照组的40.62%(P<0.05);观察组住院时间、住院医疗费用均显著低于对照组(P<0.05)。结论升降白虎汤联合胸腺肽α_1治疗老年重症肺炎患者效果显著,安全性高。Objective To investigate the clinical effect of shengjiang white tiger decoction combined with thymosin α1 on the treatment of elderly patients with severe pneumonia. Methods A total of 64 elderly patients with severe pneumonia were collected and randomly divided into observa- tion group and control group, 32 cases in each group. All the patients were treated with routine anti infection therapy, cough and asthma therapy, nutritional support and other conventional treatment. The control group was given subcutaneous injection of thymosin α1 , and the observation group was treated with additional shengjiang white tiger decotion on the basis of control group. After two weeks continuous treatment, the immune ceils （ CD3 ＋ , CD4 ＋ , CDs ＋, CD4 ＋/CD8＋ ） and serum inflamma- tory markers （ WBC, CRP and IL-6） as well as the effect and the incidence rate of adverse reactions, hospitalization time, medical cost were compared between two groups. Results After treatment, the CD3 ＋, CD4 ＋, CD8 ＋, CD4 ＋/CD8＋, WBC, CRP, IL-6 improved significantly （P 〈 0.05）. CD3 ＋ and CD4 ＋ in the observation group were significantly higher than the control group, while WBC, CRP and IL-6 were significantly lower than the control group （P 〈 0.05 ）. Patients in both groups had no obvious adverse reactions. The excellent rate of observation group was 65.62%, which was significantly higher than 40.62% of the control group （P 〈 0. 05 ）. In the observation group, hospitalization time and medical costs were significantly lower than the control group （ P 〈 0. 05 ）. Conclusion Shengjiang white tiger decoction combined with thymosin α1 is effective and safe in the treatment of elderly patients with severe pneumonia.
