摘 要:目的探讨某特大火灾爆炸事故住院伤员外伤伤情与受伤地点之间的关系。方法回顾性分析2015年8月12—16日某部队医院集中收治的特别重大火灾事故住院伤员临床资料,筛选入组外伤或合并外伤伤员。按照受伤地点距爆炸核心区的距离进行分组(A组:d≤0.20 km;B组:0.20 km<d≤1.00 km;C组:d>1.00 km),统计分析各组伤员伤情并进行比较。结果共入组外伤伤员80例(A组31例,B组34例,C组15例)。其中,发生或合并开放性伤害48例(39.02%)、表皮伤害40例(32.52%)、骨折伤或脱臼20例(16.26%)、颅脑创伤15例(12.20%)。A组开放性伤害明显高于B组;C组开放性伤害、表皮伤高发,表皮伤明显高于A组(P均<0.05)。各组间伤员伤情严重程度与受伤地点距爆炸核心区距离呈明显负相关(rs=-0.528,P<0.01)。结论特大火灾爆炸事故住院外伤伤员,其受伤地点距事故核心区的距离与伤情严重程度呈明显负相关,且外伤种类具有一定的规律,或可进一步提示救治机构完善相关预案,更加科学合理地积极应对。Objective To explore the relationships between the level of blast injury and site of blast in patients from explosions.Methods A retrospective study was made on patients following the explosions that had occurred between August 12,2015and August 16,2015.The relevant clinical data of patients with injury and those with compound injury were included.These cases were classified by the distance between the site where injury was inflicted and the site of blast(group A d≤0.20 km,group B 0.20 kmd≤1.00 km,and group C d〉1.00 km).Then,the characteristics and differences of the condition of patients in each group were assessed.Results There were 80 valid cases(31 in group A,34 in group B,and 15 in group C).The number of injuries with compound open wound,skin damage,fracture or dislocation,and craniocerebral trauma was 48(39.02%),40(32.52%),20(16.26%),and 15(12.20%)respectively.The number of patients with open wound and fracture in group A was larger than that in group B.The number of patients with open wound and skin damage was larger in group C than that in group A(P〈0.05).The correlation analysis showed negative relationships between the degree of injury and the distance of the site of injury from the blast site(rs=-0.528,P〈0.01).Conclusion Correlations between the degree of injury and the distance of the site of injury from the blast site are negative.Besides,there are some regularities in trauma classifications so that more effective emergency plans can be formulated to respond to the disaster.
