蚁参蠲痹胶囊治疗类风湿关节炎的系统评价与Meta分析  

Systematic Review and Meta Analysis of Yishen Juanbi Capsule (蚁参蠲痹胶囊) in Treating Rheumatoid Arthritis

摘　　要：目的系统评价蚁参蠲痹胶囊治疗类风湿关节炎的疗效及安全性。方法检索中国知网数据库、维普数据库、万方数据库、Pub Med数据库及Cochrane图书馆发表的关于蚁参蠲痹胶囊治疗类风湿关节炎的随机对照试验。评价纳入文献方法学质量,并采用Rev Man 5.0软件进行Meta分析。评价指标包括临床疗效评价、中医证候疗效评价及安全性评价。结果纳入符合标准文献4篇,涉及患者1784例,所有文献均未实施盲法,也未描述分配隐藏方法。Meta分析结果显示,蚁参蠲痹胶囊(或与其他药物联合)试验组临床疗效总有效率优于对照组[RR=1.20,95%CI(1.13,1.27)];试验组C反应蛋白[SMD=-1.03,95%CI(-1.18,-0.89)]及中医症状总积分[SMD=-0.56,95%CI(-0.78,-0.34)]改善优于对照组。除个别胃肠道反应报道,未见其他严重不良反应等安全性问题。结论蚁参蠲痹胶囊治疗类风湿关节炎临床疗效确切,且未见明显不良反应报道;对于其他指标的改善,尚需更多高质量研究做进一步分析论证。Objective To review the effect and safety of Yishen Juanbi Capsule in treating rheumatoid arthritis（ RA） systematically. Methods Randomized controlled trials about Yishen Juanbi Capsule treating RA were reviewed in China National Knowledge Infrastructure,VIP Database,Wanfang Database,Pub Med and Cochrane Library. The methodology quality of included papers was evaluated. Rev Man 5. 0 software was adopted to make Meta analysis. The evaluation indexes included clinical effect evaluation,Chinese Medicine syndrome effect evaluation and safety evaluation. Results Four papers met the standard were included,about 1784 patients. No papers used blind method,nor did the papers introduce allocation concealment method. Meta analysis showed that the total clinical efficacy in the Yishen Juanbi Capsule（ or combined with other medicine） group was superior to that in the control group[RR = 1. 20,95% CI（ 1. 13,1. 27） ]; the improvement of C-reactive protein（ CRP） [SMD =-1. 03,95% CI（-1. 18,-0. 89） ] and total score of Chinese medicine symptom [SMD =-0. 56,95% CI（-0. 78,-0. 34） ]in the treatment group was superior to that in the control group. There were no other serious adverse reactions and other safety problems except individual gastrointestinal reaction record. Conclusion Yishen Juanbi Capsule treating RA might be effective,without obvious adverse reactions. As for improving other indexes,more high-quality researches were needed to make further analysis and demonstration.
