福建省首次5例输入性黄热病中西医结合治疗

摘　　要：目的分析中西医结合治疗输入性黄热病的疗效。方法 2016年3月收治的5例输入性黄热病患者采用隔离、对症及支持治疗结合中医辨证治疗。5例患者中1例患者入院时有发热,予地塞米松注射液抗炎,奥美拉唑粉针剂抑酸保护胃黏膜及补液支持,5例患者均有不同程度肝损害,予复方甘草酸单铵粉针剂、还原型谷胱甘肽粉针剂保肝、降酶,并给予对症治疗。根据中医辨证分型辨证选择口服全成分中药颗粒剂,每日1剂,分两次冲服。1例发热患者入院时有恶寒、头身痛,厌食、恶心,舌边尖红,舌苔中后部白厚腻,脉浮数,辨证湿热郁阻,方选甘露消毒丹加金银花、连翘及佩兰等。连服5剂。其他患者入院时无发热,感轻度乏力,饮食一般,舌淡,苔少,脉细。余邪未净证,方选薏苡竹叶汤加金银花、太子参。连服7剂。观察患者体温、临床症状及实验室结果。结果所有患者均达到体温正常,临床症状缓解,均符合出院标准,痊愈出院。结论中西医结合治疗黄热病可以取得良好疗效。Objective To analyze the effect of integration of Chinese and western medicine in treating imported yellow fever. Methods Five imported yellow fever patients treated with isolation, symptomatic therapy and syndrom-differentiation based Chinese medicine in Mar 2016 were introduced. One of the five cases had a fever on admission,which was given Dexamethasone injection for anti-inflammatory,omeprazole powder injection for acid suppression to protect gastric mucosa,as well as fluid infusion. All five cases had hepatic injury to various degrees. Compound ammonium glycyrrhetate injection powder,reduced glutathione powder injection were given to protect liver and reduce enzyme,as and they were also given symptomatic treatment. Total-component Chinese drug granules were taken orally based on syndrome differentiation,one dose daily,divided into twice. One patient with fever on admission had aversion to cold,headache and pantalgia and was anorectic and nauseous. The tip and edges of the tongue were red,the postmedian fur was white,thick and greasy and the pulse was floating and rapid. The syndrome differentiation was damp-heat stagnation. Ganlu Xiaodu Micropills（ 甘露消毒丹） plus Jinyinhua [Flos Lonicerae],Lianqiao[Fructus Forsythiae],Peilan [Herba Eupatoriae]and so on was given for 5 consecutive doses. The other patients felt slightly weak and had a common appetite on admission. The tongue was pale,the fur was scanty and the pulse was thready. The syndrome type was pathogenic factors residue. Yiyi Zhuye Decoction（ 薏苡竹叶汤） plus Jinyinhua [Flos Lonicerae] and Taizishen [Radix Pseudostellaria] was given for 7 doses. The body temperature,clinical symptoms and laboratory result of the patients were observed. Results The body temperature of all patients was normal and the clinical symptoms alleviated. All cases met the discharge criterion. Conclusion Integration of Chinese and western medicine in the treatment of yellow fever might have good result.
