灯盏花素联合奥曲肽对重症急性胰腺炎患者炎症因子及血管活性物质的影响

摘　　要：目的:探讨灯盏花素联合奥曲肽对重症急性胰腺炎(SAP)患者炎症因子及血管活性物质的影响。方法:选取SAP患者72例进行研究,随机分为观察组和对照组,两组入院后均及时给予胃肠减压、禁饮、禁食、预防感染、纠正酸碱平衡及电解质紊乱等治疗,对照组给予奥曲肽注射液0.6mg入生理盐水48mL微泵静脉注射,1次/12h,观察组在此基础上联合灯盏花素注射液20 mg入5%葡萄糖250mL静脉滴注,1次/d,两组均连用10d进行疗效评价。两组抽取治疗前后清晨空腹外周静脉血,采用ELISA法测定C反应蛋白(CRP)、肿瘤坏死因子-α(TNF-α)、白细胞介素-8(IL-8)、白细胞介素-15(IL-15)、一氧化氮(NO)、血管性血友病因子(vWF);采用放射免疫法测定血浆血栓素B_2(TXB_2)、6-酮-前列环素(6-keto-PGF_(1α))、内皮素(ET)浓度;采用全自动血液流变仪测定红细胞压积、红细胞聚集指数、全血低切黏度、全血高切黏度、血浆黏度。结果:观察组治疗后红细胞压积、红细胞聚集指数、血浆黏度、全血低切黏度、全血高切黏度水平低于对照组(P<0.05);观察组治疗后血清CRP、IL-8、IL-15、TNF-α水平低于对照组(P<0.05);观察组治疗后血浆ET、TXB_2、vWF水平低于对照组,6-keto-PGF_(1α)、NO水平高于对照组(P<0.05)。结论:灯盏花素联合奥曲肽可有效抑制SAP患者炎症反应,改善血管内皮功能,改善血液流变学及微循环障碍,有效促进疾病的恢复。Objective：To explore the effect of breviscapine in combined with octreotide on the inflammatory reaction and vasoactive substances in patients with severe acute pancreatitis（SAP）.Methods：A total of 72 patients with SAP who were admitted in our hospital from January,2014 to November,2016 were included in the study and randomized into the observation group and the control group.The patients in the two groups were given gastrointestinal decompression,water deprivation,fasting,infection prevention,and acid-base imbalance and electrolyte disturbance correcting.The patients in the control group were given micropump intravenous injection of octreotide（0.6mg）and normal saline（48mL）,1time/12 h.On this basis,the patients in the observation group were given intravenous injection of breviscapine（20mg）and 5% glucose（250mL）,1time/d.After 10d-treatment,the efficacy was evaluated.The morning fasting peripheral venous blood before and after treatment in the two groups was collected.ELISA was used to detect CRP,TNF-α,IL-8,IL-15,NO,and vWF.RIA was used to detect the plasma TXB2,6-keto-PGF1α,and ET concentration.The full automatic blood rheometer was used to detect HCT,EAI,whole blood low shear viscosity,whose blood high shear viscosity,and plasma viscosity.Results：HCT,EAI,whole blood low shear viscosity,whose blood high shear viscosity,and plasma viscosity after treatment in the observation group were significantly lower than those in the control group（P〈0.05）.The serum CRP,IL-8,IL-15,and TNF-αlevels after treatment in the observation group were significantly lower than those in the control group（P〈0.05）.The plasma ET,TXB2,and vWF levels after treatment in the observation group were significantly lower than those in the control group（P〈0.05）,while 6-ketoPGF1α and NO levels were significantly higher than those in the control group（P〈0.05）.Conclusions：Breviscapine in combined with octreotide can effectively inhibit the inflammatory reaction in patients with SAP,improve the vascu
