尖锐湿疣感染患者不同病程CD8^+、CD4^+ T淋巴细胞与人乳头状瘤病毒感染的相关因素分析

摘　　要：目的分析尖锐湿疣感染患者不同病程外周血T淋巴细胞亚群的表达与人乳头状瘤病毒HPV感染的相因素。方法将2015年10月至2016年10月收治的72例尖锐湿疣患者作为本次研究的对象,根据病程分为初发组42例,复发组30例,选同期进行健康体检的50名健康人员为对照组,检测三组受试者外周血T淋巴细胞亚群,分析各组人员HPV感染类型及外周血T淋巴细胞亚群的表达,同时分析尖锐湿疣HPV感染者阳性率。结果对照组和初发组CD4^+/CD8^+、CD4^+值明显高于复发组,差异显著(P<0.05),复发组CD8^+值显著高于初发组与对照组(P<0.05);72例患者采用PCR检测HPV均呈现为阳性,HPV6/11型感染22例,HPV6/18型感染23例,混合感染为27例。三组受试者外周血T淋巴细胞亚群无显著差异(P>0.05)。结论尖锐湿疣患者随着病程增长,造成人体免疫细胞功能下降明显,导致尖锐湿疣复发率升高。Objective To analyze the related factors of expression of subsets of T lymphocytes in peripheral blood and HPV infection in patients with condyloma acuminatum. Methods Seventy two patients with condyloma acuminatum during October 2015 to October 2016 were allocated in this study,and they were divided into primary group（ 42 cases） and recurrent group（ 30 cases） according to the course of disease,and50 healthy persons for physical examination were selected as control group,and T lymphocyte subsets in perippheral blood of these 3 groups were analyzed,at the same time the types of HPV infection and peripheral subsets of T lymphocytes and the positive rate of HPV in patients infected with condyloma acuminatum were analyzed. Results The figures of CD4＋/CD8＋and CD4＋in control group and primary group were significantly higher than those of recurrence group,with significant difference（ P〈0. 05）,they were significantly higher than those of primary group and control group,the figures of CD8＋in rrecurrent group were significantly higher than those of primary group and control group（ P〈0. 05）. HPV was positive in all 72 patients by PCR,22 cases infected with type HPV6/11 infection,23 cases infected with type HPV6/18 infection,and 27 cases with mixed infection. There was no significant difference in T lymphocyte subsets in peripheral blood among these three groups（ P〈0. 05）. Conclusion The function of human immunocytes was significantly decreased following the prolongation of course of condyloma acuminatum,resulting in significant decrease of immunological function of human immune cells,leading to high recurrence rate of condyloma acuminatum.
