摘 要:目的对医院输血科管理中持续质量改进的应用效果进行探讨。方法整群选取该院2015年1—9月的364份输血病历作为对照组,总结输血管理各个环节存在的问题,采用持续质量改进措施,整群收集2016年1—9月的372份输血病历作为观察组,比较两组输血病历记录的缺陷发生率、质量管理评分以及医务人员和患者对输血管理工作的满意度。结果 (1)经比较,观察组病案首页、输血医嘱、病程记录、护理记录、输血记录、输血筛查、输血会诊以及合理用血记录缺陷率分别为8.87%、6.18%、9.14%、3.49%、2.96%、1.34%、0.00%和1.08%,均显著低于对照组的17.31%、12.64%、20.88%、12.36%、11.26%、4.67%、13.73%和6.04%,组间差异有统计学意义(P<0.05)。(2)经比较,观察组的管理质量评分为(90.24±5.37)分,高于对照组的(81.34±3.07)分,组间差异有统计学意义(P<0.05)。(3)经比较,观察组的满意度为95.00%,高于对照组的86.00%,组间差异有统计学意义(P<0.05)。结论在医院输血科管理中应用持续质量改进可以降低病历记录的缺陷,提高病历质量的合格率,提升输血科管理工作的规范性,提高医务人员和患者的满意度,适合进行临床推广。Objective To observe the application effect of continuous quality improvement in the management in the department of blood transfusion in the hospital. Methods Group selection 364 transfusion medical records from January 2015 to September 2015 were selected as the control group, and the issues in the blood transfusion management were summarized,and the continuous quality improvement measures were adopted, 372 transfusion medical records from January 2016 to September 2016 were selected as the observation group, and the incidence rate of shortcomings of blood transfusion medical records, quality management score and satisfactory degree of medical staff to the blood transfusion management work were compared between the two groups.] Results The comparison showed that the differences in the medical record home page,blood transfusion doctor's signs, disease course records, nursing records, blood transfusion records, blood transfusion screening, consultation and shortcoming rate of rational blood use records between the observation group and the control group were statistically significant(8.87%, 6.18%, 9.14%, 3.49%, 2.96%, 1.34%, 0.00%, 1.08% vs 17.31%, 12.64%,20.88%, 12.36%, 11.26%, 4.67%, 13.73%, 6.04%)(P〈0.05), the management quality score in the observation group was higher than that in the control group [(90.24±5.37)points vs(81.34±3.07)points], and the difference was statistically significant(P〈0.05), and the satisfactory degree in the observation group was higher than that in the control group(95.00% vs86.00%), and the difference was statistically significant(P〈0.05). Conclusion The application of continuous quality improvement in the management in the department of blood transfusion in the hospital can reduce the shortcomings of medical records, improve the qualified rate of medical record quality, standardization of management work in the department of blood transfusion and satisfactory degree of medical staff and patients, which is suitable for clinical promotion.
