摘  要:目的探讨腔内手术离断部分小梁结构对不稳定膀胱(overactive bladder,OAB)犬尿动力学的影响。方法将9只不稳定膀胱犬模型随机分成实验组和对照组(2组),实验组(n=5)行解除膀胱出口梗阻,再行膀胱逼尿肌小梁连续全层离断术,对照组(n=4)仅解除膀胱颈口梗阻。2组于术后3个月、6个月分别行最大尿流率、逼尿肌压力、最大膀胱容量、膀胱最大顺应性、残余尿量等尿动力学检测。结果实验组术后第3个月的最大尿流率低于对照组(P<0.05),术后第6个月2组间最大尿流率差异无统计学意义(P>0.05)。实验组术后第3、6个月最大膀胱容量明显大于对照组(P<0.05)。2组犬模型术后第3、6个月逼尿肌压力、膀胱顺应性、残余尿量差异无统计学意义(P>0.05),实验组有1只犬发生膀胱穿孔不良事件。结论腔内手术离断部分逼尿肌小梁纤维化结构,风险及创伤较小,对提高最大膀胱容量效果明显,可为开展人类治疗OAB提供理论基础及平台,或不失为治疗OAB临床症状的一种新方法。Objective To evaluate the impact of Endoscopic surgery on the Urodynamics of canine unstable bladder. Methods Dogs with Unstable bladder were randomly divided into two groups, one group with rows relieve bladder outlet obstruction, followed by cut a continuous full-thickness detrusor trabecular, whereas another group only undergone lift of the bladder neck obstruction. The two groups in the first three months and six months after surgery were subjected to urodynamic testing. Results The third month of the maximum flow rate in the experimental group was shown to be significantly lower than that of the control group (P 〈C0.05) , But no significant difference was observed between the two groups at the sixth month (P 〉0.05). The maximum bladder capacity of the experimental group was presented to be significantly larger than that of the control group at the third and sixth month, respectively (P 〈C0.05). However, no significant difference was observed between the two groups in terms of the detrusor pressure, bladder compliance, and residual urine volume at the third and sixth month. Notably, it bears mentioning that there was an experimental canine whose bladder perforation occurred. Conclusion Our study showed that there was relatively small risk and trauma in the endoscopic surgery cutting part fibrosis trabecular structure in bladder, which could provide a theoretical basis and platform for the development of human therapeutic OAB, or could be used as a new kinds of methods treatment of OAB symptoms.
