93例下腔静脉滤器预防致死性肺栓塞的临床研究

摘　　要：目的探究下腔静脉滤器在预防致死性肺栓塞上临床应用价值。方法方便抽取2013年1月—2016年3月该院血管外科收治的93例急性下肢深静脉血栓形成患者,均置入下腔静脉滤器,并接受5~7 d的导管接触性溶栓治疗后行造影,若患肢血栓清除则取出下腔静脉滤器,若患肢血栓未完全清除则采用大腔导管吸栓或继续导管接触性溶栓2~3 d再行造影,若血栓仍无法完全清除则长期留置下腔静脉滤器,并接受随访,该组共3例患者长期留置下腔静脉滤器,对所有患者的临床治疗效果及致死性肺栓塞发生率进行回顾性分析。结果 93例下肢深静脉血栓患者置入下腔静脉滤器均成功,置入成功率达到100%。经导管接触性溶栓治疗后,其中治愈90例,93例患者中未发生致死性肺栓塞,治疗效果良好。结论对下肢深静脉血栓患者在行溶栓治疗前置入下腔静脉滤器,可有效预防致死性肺栓塞,提升患者的生存质量,值得在临床治疗上进一步应用和推广。Objective To study the clinical application value of inferior vena cava filter in preventing lethality pulmonary embolism. Methods 93 cases of patients with acute deep vein thrombosis of lower limb admitted and treated in our hospital from January 2013 to March 2016 were convenient selected and given the inferior vena cava filter, and the patients were given the radiography after the catheter directed thrombolysis treatment of 5 d to 7 d, if the thrombus of affected limb was removed, the inferior vena cava filter was taken out, if the thrombus of affected limb was not totally removed, the patients were given the radiography after adopting the large cavity catheter thrombus aspiration or catheter directed thrombolysis of 2 d to 3 d, if the thrombus of affected limb was still not totally cleared, the inferior vena cava filter were indwelled for a long term, and the patients were followed up, in this group, there were 3 cases with long-term indwelling of inferior vena cava filter, and the clinical treatment effect and incidence rate of pulmonary thromboembolism of all patients were retrospectively analyzed. Results Of the 93 cases with deep venous thrombosis, the implementation success rate reached 100%, and 90 cases were cured after the catheter directed thrombolysis treatment, and there was no lethality pulmonary embolism and the treatment effect was good. Conclusion The inferior vena cava filter for patients with deep venous thrombosis before the intravenous thrombolytic therapy can effectively prevent the lethality pulmonary embolism, improve the survival quality of patients, which is worth further application and promotion in clinical treatment.
