摘　　要：通过对国际、国内慢性病及慢性病高风险人群的管理情况总结和概括认为,慢性病及高风险人群的管理主要依靠社区管理实现,以国际、国内干预项目为契机,研究制订适合中国慢性病高风险人群的管理方案十分迫切,应依据中国国情发挥好社区对慢性病高风险人群的管理。Based on the summary of international and domestic situation about the management of chronic diseases and chronic at high risk population the management rely mainly on community management implementation, with domestic and international intervention project as an opportunity, it is very urgent to study and formulate suitable plan for people at high risk management in China, research should be based on the situation of our country play a good developing chronic disease management through community disease for people at high risk.
