醒脑开窍针刺法治疗阻塞性睡眠呼吸暂停低通气综合征临床观察

摘　　要：目的:观察醒脑开窍针刺法治疗阻塞性睡眠呼吸暂停低通气综合征的临床效果。方法:选取本院收治的阻塞性睡眠呼吸暂停低通气综合征患者100例,随机分为对照组和观察组各50例。对照组给予经鼻持续气道内正压通气治疗,观察组在对照组治疗的基础上加用醒脑开窍针法,连续治疗4周。比较两组患者的临床疗效及相关指标的变化。结果:观察组有效率为88.0%,对照组有效率为70.0%,两组有效率比较有显著性差异(P<0.05);观察组经治疗后的AHI、夜间微觉醒次数与对照组比较明显降低,而LSp O2水平则显著高于对照组,差异性显著(P<0.05);观察组治疗过程中的嗜睡程度改善优于对照组,其ESS评分显著低于对照组,差异性显著(P<0.05);观察组患者血清炎性因子hsCRP、TNF-α、IL-6水平较对照组显著降低,差异性显著(P<0.05)。结论:醒脑开窍针刺法治疗阻塞性睡眠呼吸暂停低通气综合征疗效肯定,利于缓解临床症状,减轻微觉醒次数,提高最低血氧饱和度,改善嗜睡,其机制与改善缺氧、调控血清炎性因子水平等有一定相关性。Oijeetive： To observe the clinical effects of consciousness-restoring and orifices-opening acupuncture on ohstructive sleep apnea hypopnea syndrome （OSAHS）. Methods： Totally 100 patients with OSAHS were randomized into control group and observation group, with 50 cases in each group. Control group were given continuous positive ainvay pressure （CPAP） while ob- servation group were additionally given consciousness-restoring and orifiees-npening acupuncture, 4 weeks of continuous treatment fir both groups. The clinical effects and changes of relevant indicators in the two groups were compared. Results： The effective rate was 88.0% in observation group and 70.0% in control group; the difference had statistical significanee（ P 〈 0.05）. After treatment, the level of AHI, times of aruusals in night in observation group declined more than these in control group, hut the level of LSpO2 in ohservation group was higher than that in control group; the difference was significant （ P 〈 0.05）. The im- pruvement of drowsiness in observation group was markedly superior lu that in control gruup, and the ESS in observation group was greatly lower than that in control group, with significant difference （P 〈 0.05）. The levels of hs-CRP, TNF-ctand IL-6 in observation group declined more than these in control group; the difference was significant （P 〈0.05 ）. Conclusion： Conscious- hess-restoring and orifices-opening aeupuncture has definite therapeutic effeels on OSAHS, which can relieve the clinical symp- toms, reduce the awaking times, increase the minimum oxyhemoglobin saturation and enhance drowsiness. The mechanism has certain relationship with the improvement of auoxia and regulation of the level of serum inllammatory factors.
