针刺结筋病灶点治疗慢性踝关节扭伤随机对照研究

摘　　要：目的:观察比较毫针针刺结筋病灶点与经穴治疗慢性踝关节扭伤的临床疗效。方法:将符合纳入标准的120例慢性踝关节扭伤患者,随机分为结筋病灶点组和经穴组,每组各60例。结筋病灶点组患者先进行触诊,常见结筋病灶点为解溪次、丘墟次、下丘墟、中封次、商丘次、冲阳次、昆仑次、光明次、申脉次、京骨次、束骨次、太溪次、泉生足次、女膝次、照海次、公孙次等。标记、消毒,每次选取5~8个进行毫针针刺,留针30min。1次/d,5次/周,共4周;经穴组毫针针刺解溪、昆仑、申脉、照海、丘墟,留针30min。1次/d,5次/周,共4周。记录2组患者治疗前后的VAS分值与踝关节功能分值,并进行疗效评价。结果:(1)结筋病灶点组疗效明显优于经穴组(P<0.01);(2)在减轻疼痛方面,结筋病灶点组明显优于对照组(P<0.01);(3)在改善踝部功能方面,结筋病灶点组明显优于对照组(P<0.01)。结论:毫针针刺结筋病灶点治疗慢性踝关节扭伤可以更好的缓解疼痛、改善功能,疗效显著,值得临床推广应用。Objective： To observe the curative effect of acupuncture at acupoints of acupuncture points and acupoints on chronic ankle sprain. Methods： 120 cases of chronic ankle sprain were randomly divided into two groups： The focal group and the control group. Each group of 60 cases. Tendon focal points are Jiexici, Qiuxuci, Xiaqiuxu, Zhongfengci, Shangqiuci, Chongyangci, Kunlunci, Guangmingci, Shenmaici, Jingguci, Shuguci, Taixiei, Quanshengzuci, Nvxici, Zh- aohaici, Gongsunci, et al. Mark, then disinfection, each selected 5 to 8 for acupuncture needle, needle 30min. 1 times/ d, 5 times/week, a total of 4 weeks. Meridian acupuncturing acupoint meridian, Kunlun, Jiexi, Zhaohai, Qiuxu, needle retention 30rain. 1 times/d, 5 times/week, a total of 4 weeks. The scores of VAS and ankle function were recorded before and after treatment, and the curative effect was evaluated. Results： （1） The curative effect of focal group was better than that of acupoints group （P〈0.01）; （2） In alleviating the pain, the focal group was better than the control group （P〈0.01） in improving the function of the ankle, focal t group was significantly better than the control group （P〈0.01）. Conclu- sion： Acupuneturing at tendon focal points for treatment of chronic ankle sprain can relieve pain, improve function, the effect is significant, it is worthy of clinical application.
