活血灵方复合低分子肝素钙预防髋关节置换术后下肢深静脉血栓形成临床研究

摘　　要：目的:探讨髋关节置换术后活血灵方口服复合低分子肝素钙预防下肢深静脉血栓的临床疗效与安全性。方法:将符合纳入标准的60例拟行髋关节置换术患者,按随机字母表法分为对照组和观察组,每组各30例,对照组应用低分子肝素钙,观察组在对照组基础上加用活血灵方,2组治疗周期均为14d,观察比较治疗后2组患者下肢深静脉血栓形成发生率、凝血5项及患肢周径变化。结果:观察组患者D-二聚体、凝血酶原时间(PT)、凝血酶时间(TT)、凝血酶时间(APTT)、纤维蛋白原(FIB)术后21d与术后1d比较差异显著(P<0.05),2组患者DVT发生率和DVT可疑发生率无统计学差异(P>0.05),术后21d观察组患肢周径与术后1d比较,差异有统计学意义(P<0.05),且对照组患者术后21d患肢仍轻微肿胀。结论:活血灵方复合低分子肝素钙能有效预防髋关节置换术后患者下肢深静脉血栓的发生,无临床不良反应,安全可靠,可进行临床推广使用。Objective： To investigate the clinical efficacy and safety of Huoxueling decoction combined with low molecular weight heparin calcium in the prevention of deep venous thrombosis of lower extremities after hip arthro- plasty. Methods： 60 patients who underwent hip arthroplasty were randomly divided into control group and observation group （30 cases）. The control group was treated with low molecular weight heparin calcium. The observation group was treated with Huoxueling decoction, the treatment group was treated for 14 days. The incidence of deep venous throm- bosis, coagulation and limb circumference were observed and compared between the two groups. Results： There were significant differences in the levels of D-dimer, prothrombin time （PT）, thrombin time （TT）, thrombin time （APTT）and fibrinogen（FIB）（P〈0.05）.There was no significant difference in the incidence of DVT and suspicious DVT between the two groups（P〉0.05）, and there was significant difference between the observation group and the postoperative day I, and the control group of patients 21 days after the limb is still slightly swelling. Conclusion： Huoxueling recipe com- pound low molecular weight heparin calcium can effectively prevent the occurrence of deep venous thrombosis of lower limbs in patients after hip arthroplasty, without clinical adverse reaction, safe and reliable, and can be popularized and used clinically.
