脑血管病患者死亡风险预警评分调查及影响因素分析

摘　　要：目的 探讨脑血管病患者的死亡风险预警状况及影响因素.方法 选择2015年1-12月在上海市某三甲医院住院治疗过程中死亡的123例脑血管病患者为研究对象,采用自行设计调查表及英国国家早期预警评分（NEWS）对患者死亡前24 h的临床资料进行调查,对影响预警评分的原因进行分析.结果 本组脑血管疾病患者NEWS得分平均为（7.18±2.40）分,其中低危组0例;中危组53例,占43.1%,平均（4.87±1.09）分;高危组70例,占56.9%,平均（8.93±1.49）分.多元线性回归分析显示意识障碍持续的时间、留置管个数、压疮与否对NEWS评估病情严重程度有影响（P〈0.05）.结论 脑血管病患者意识障碍持续的时间、留置管个数、压疮与否是影响NEWS评估病情严重程度的影响因素.对脑血管病患者进行针对性的护理评估,有益于促进患者的不良结局的转归.Objective To investigate the risk of death and its influencing factors in patients with cerebrovascular disease.Methods Totally 123 cases of death patients with cerebrovascular disease in Shanghai from January to December of 2015 were selected as the research subject. Self-designed questionnaire and the National Early Warning Score （NEWS） were used to investigate the clinical data of 24 h before the death and analysis the causes affecting the early warning score.Results The average score of NEWS in patients with cerebrovascular disease was （7.18±2.40）,among which the number of low risk group was 0 （0%）,the number of moderate risk group was 53 （43.1%） with the average score of （4.87±1.09） and the number of high risk group was 70 cases （56.9%） with average score of （8.93±1.49）. Multiple linear regression analysis showed that the duration of disturbance of consciousness,the number of indwelling tube,pressure sores or not affect the NEWS assessment of disease severity （P〈0.05）.Conclusions The duration of disturbance of consciousness,the number of indwelling tube and the pressure sore or not are the influencing factors of the severity of the disease in the cerebrovascular disease patients. Targeted nursing assessment in patients with cerebrovascular disease help to promote the adverse outcomes of patients.
