摘 要:目的 探讨回馈教学对冠心病患者锻炼依从性的影响.方法 选择2014年6-12月在解放军总医院304临床部老年心内科治疗的符合条件的冠心病患者38例为对照组,选择2015年1-7月在解放军总医院304临床部老年心内科治疗的符合条件冠心病患者41例为观察组.对照组接受常规健康教育,观察组在对照组常规护理的基础上进行回馈教学护理模式.比较两组患者干预后患者心境状态、锻炼依从性、锻炼知识掌握情况及病情改善情况.结果 干预前,心境状态中的7个维度两组差异无统计学意义(P〉0.05);干预后在7个维度上的改善情况观察组均优于对照组,差异有统计学意义(t值分别为4.658、4.251、3.318、6.123、4.587、5.487、4.982;P〈0.01);观察组患者在干预后,心境状态明显优于干预前(P〈0.05),对照组患者在紧张、抑郁、混乱方面有改善(P〈0.05),在其他方面无改善(P〉0.05).干预前,两组在锻炼依从性、锻炼知识合格率和每天锻炼时间方面差异无统计学意义(P〉0.05).教育后观察组与对照组在锻炼依从性、锻炼知识合格率和每天锻炼时间方面较教育前均有明显提升(P〈0.05);但观察组的改善情况优于对照组(t值分别为3.965、4.521、4.689;P〈0.01).观察组患者共改善21例,改善率为51.22%;对照组患者改善6例,改善率为15.79%,两组差异有统计学意义(x2=10.235,P〈0.05).结论 对冠心病患者在常规健康教育的基础上实施回馈教学教育模式,能提高患者的锻炼依从性,改善病情状况和心境状态,从而提高生活质量.Objective To explore the effect of feedback teaching on the exercise compliance of the patients with coronary heart disease.Methods Thirty-eight coronary heart disease (CHD) patients who met the criteria and were treated in the department of geriatric cardiology in 304 clinical department of People's Liberation Army General Hospital from June 2014 to December 2014 were included into the control group. Forty-one coronary heart disease patients who met the criteria and were treated in the department of geriatric cardiology in 304 clinical department of People's Liberation Army General Hospital from January 2015 to July 2015 were included into the observation group. The patients in the control group received the routine health education. The patients in the observation group received the feedback teaching nursing model based on the routine nursing. The state of mind,the exercise compliance,the grasp of exercise knowledge and the situation of illness improvement of the patients after the intervention were compared between the two groups.Results Before the intervention, no significant difference of the seven dimensions in the state of mind was found between the two groups (P〉0.05).After the intervention the improvement situations of seven dimensions were all better in the observation group compared to the control group(t=4.658,4.251,3.318,6.123,4.587,5.487,4.982;P〈0.01). After the intervention the state of mind of the patients in the observation group was better than that before the intervention (P〈0.05). In the control group,patients were significantly improved in tension,depression,confusion (P〈0.05).In other respects no improvement was found in the patients in the control group (P〉0.05).
