改良瘘管结扎术治疗肛瘘的疗效观察s

摘　　要：为了观察改良瘘管结扎术治疗肛瘘的临床疗效，将2014年1月至2016年6月，上海复旦大学附属华山医院（45例）和华山医院宝山分院肛肠科（35例）收治的80例经括约肌间肛瘘手术患者，按随机数字表法分为观察组（改良瘘管结扎术）和对照组（传统肛瘘切除缝合术），每组各40例。观察术后出血、术后疼痛，创面渗液、肛缘水肿、愈合天数、治愈率、复发率、肛门功能测试等指标。结果显示，观察组治愈率为97．5％（39／40），显著高于对照组的85．0％（34／40），差异有统计学意义（P〈0．05）。观察组手术时间（39．82±3．23）min、创口面积（1．50±0．25）cm2、创面愈合时间（11．11±1．13）d、住院时间（7．23±1．32）d，均显著优于对照组[（43．12±5．20）min、（3．10±0．51）cm2、（15．30±2．13）d、（10．34±4．20）d]，差异均有统计学意义（均P〈0．05）。与治疗前相比，2组治疗后肛门失禁渗出均明显下降，但观察组显著低于对照组（P〈0．05）。结果表明，改良瘘管结扎术与传统肛瘘切除缝合术相比，经济、安全、痛苦小，恢复快，治愈率高，且肛门括约肌功能保护好。This study was to observe the clinical effect of modified ligation of fistula tract for treating anal fistula,randomly divided 80 cases of fistula who admitted in Baoshan branch of Huashan Hospital （ n = 35） as well as Huashan hospital（ n =45） and were subject to procedure of intersphincter fistula tract into observation group （as modifed ligation） and control one （ as routine resection＆ suture for fistula in ano）, 40 cases for each; observed both groups following indexes as postoperative bleeding, pain, exudation from wound surface, ano edge edema, healing time （days）, curative rate recurrence rate, anal function measure- ment, etc.As results, in curative rate, operative time, wound area, wound-surface healing time, and hospital- ization time, observation group was all significantly superior to control one： 39/40 （97.5%） vs 34/40 （85.0%）,（39.82±3.23） min vs （43.12±5.20） min,（1.5±0.25） cmz vs （3.10±0.51） cme,（11.11±1.13） d vs （15.3±2.13） d,（7.23±1.32） d vs （10.34±4.20） d, （all P 〈0.05） ;what＇s more,compared with before treatment both group＇s postoperative anal incontinence and exudation had all significantly lower, but obser- vation group was more lower（ P 〈0.05）.Results show that comparing two procedures modified procedure is economic, safe, and of less pain, faster recovery, high curative rate, as well as better protecting anal sphincter＇s function.
