美沙拉秦口服联合手术治疗克罗恩病合并肛周脓肿疗效观察

摘　　要：为探讨克罗恩病合并肛周脓肿的合理治疗方法，回顾性分析2013年1月至2016年6月，江苏省常州市第二人民医院收治的克罗恩病合并肛周脓肿28例患者的临床资料。采用脓肿切开引流联合美沙拉秦口服的综合疗法。治疗后随访3个月，观察总体有效率，对比治疗前后疾病活动指数（HBI）、白细胞（WBC）、C型反应蛋白（CRP）、血沉（ESR）等指标。结果显示，28例患者中，缓解19例（67．9％），改善7例（25．0％），无效2例（7．1％）。复发5例（17．9％）。愈合时间平均（3．2±0．7）个月。手术后HBI指数、WBC、ESR、CRP较术前均明显下降（P〈0．05）。结果表明，内外科联合治疗克罗恩病合并肛周脓肿疗效明显。In order to explore for a rational therapy for Crohn＇s disease complicated with perianal abscess, authors retrospectively analysed the clinical data of 28 patients with above diseases who admitted in author ＇s hospital from Jan. 2013 to Jun. 2016 and received a combination therapy of dissecting abscess to make drainage and oral administration of mesalazine,after treatment through a follow-up of 3 months observed their whole effective rate,and compared Harvey-Bradshaw Index（HBI）, WBC, CRP and ESR, etc between before and after treatment. As results, among the 28 cases 19 got alleviation （67.9%）, 7 got improved （25.0%） ,2 were of non-response（7.1%） ;5 got recurrence（17.9%） ;averaged healing time was （3.2±0.7） months;after operation HBI,WBC,ESR and CRP were all decreased significantly（ P 〈0.05）.Results show that combination therapy for complicated with perianal abscess of Crohn＇s disease can achieve great effect.
