摘  要:目的比较单侧跟痛症患者患侧及健侧的足底压力分布情况。方法选择2015年11月~2016年11月南方医科大学中西医结合医院就诊的单侧跟痛症30例患者,应用平板式足底压力测试系统(Footscan USB2)测量患者患侧与健侧足底10个区域的峰值压力及冲量值。结果患者的患侧第1跖骨底(M1)、足跟外侧底(HL)和足跟内侧底(HM)的峰值压力均高于健侧(P<0.05);而两侧第1趾骨底(T1)、第2~5趾骨底(T2~T5)、第2~5跖骨底(M2~M5)和足中部底(MID)峰值压力均无显著性差异(P>0.05)。患侧M1、HL、HM的冲量均高于健侧(P<0.05);而M5和MID的冲量均则低于健侧(P<0.05)。结论跟痛症患者患侧足底压力分布异常,患侧的足跟处压力较健侧增加,且足跟处的压力中心轨迹发生弯曲并向内侧方偏移,而前足的足底压力也由中心向内下侧方偏移;患侧足跟内侧底冲量高于健侧,考虑与足底跖腱膜炎及跟垫病变有关。测量分析足底压力分布,有助于预防和早期发现跟痛症并进行早期治疗。Objective To explore the plantar pressure distribution of the affected and healthy side in the patients with unilateral calcaneodynia. Methods From November 2015 to November 2016, in Hospital of integrated TCM and West- ern Medicine, Southern Medical University, 30 patients diagnosed with unilateral caleaneodynia, they were tested with plank test system for plantar pressure (Footscan USB2), including the maximum force and impulse of 10 zones of the feet. Results Compared with the healthy feet, the first metatarsal bone (M1), lateral heel (HL) and medial heel (HM) of the affected feet increased (P 〈 0.05), but there were no significant differences between the affected side and the healthy side in the maximum force of the first toe bone (T1), 2nd-5nd toe bones (T2-T5), 2nd-5nd metatarsal bone (M2- M5) and midfoot (MID). The impulse of M1, HL and HM in the affected feet increased (P 〈 0.05), while the impulse of M5 and MID reduced (P 〈 0.05). Conclusion The plantar pressure distribution of the affected side in ealcaneodynia patients is abnormal. The heel pressure increase and the heel pressure center shift inward, while the forefoot pressure center shift inward and downward. The impulse increase under HM, which may be related to plantar fasciitis and heel pad lesions. Measurement and analysis of plantar pressure distribution is helpful for prevention and early detection of calcaneodynia and helpful for patients with early treatment.
