摘 要:目的探讨综合康复训练对慢性阻塞性肺疾病患者运动功能、肺功能、生活质量及日常生活活动能力的影响。方法遵循自愿原则,选择2015年2月~2016年10月重庆三峡中心医院呼吸科接受综合康复训练的48例慢性阻塞性肺疾病患者为观察组,另48例未接受综合康复训练的慢性阻塞性肺疾病患者为对照组。训练4周,分别测评训练前后两组患者的运动功能指标、肺功能指标、生活质量指标及日常生活活动能力指标,并进行比较分析。结果干预前,两组患者运动功能指标、肺功能指标、生活质量评分及日常生活活动能力评分比较,差异无统计学意义(P>0.05)。干预后,两组患者6 min步行距离、徒手肌力六级分法、用力肺活量、第一秒用力呼气容积、第一秒用力呼气容积/用力肺活量和日常生活活动能力均高于干预前,呼吸困难指数和生活质量评分均低于干预前,差异有统计学意义(P<0.05)。干预后,观察组呼吸困难指数和生活质量评分均明显低于对照组,其他指标均明显高于对照组,差异有统计学意义(P<0.05)。结论综合康复训练能够改善稳定期慢性阻塞性肺疾病患者的运动功能、肺功能,提高患者生活质量,提高日常生活活动能力。Objective To explore the effect of comprehensive rehabilitation training on motor function, lung function, quality of life and daily living ability of patients with chronic obstructive pulmonary disease. Methods Followed the voluntary, 48 patients with chronic obstructive pulmonary disease recieved comprehensive rehabilitation training in De- partment of Respiration, Chongqing Three Gorges Central Hospital from February 2015 to October 2016 were selected as observation group, the other 48 patients with chronic obstructive pulmonary disease who did not receive comprehen- sive rehabilitation training were selected as control group. After 4 weeks training, the indexes of motor function, pul- monary function, quality of life and activities of daily living of the patients in two groups before and after training were measured and compared. Results Before intervention, there was no significant difference between two groups in motor function, pulmonary function, quality of life score and daily living ability score (P 〉 0.05). After intervention, six min- utes walking distance, manual muscle, forced vital capacity, forced expiratory volume in the first second, forced expira- tory volume in the first second and forced vital capacity ratio and activities of daily living were higher than before inter- vention in two groups, dyspnea index (mMRC) and quality of life score were lower than before intervention in two groups, the differences were statistically significant (P 〈 0.05). After intervention, mMRC and quality of life score in observation group were significantly lower than those in control group, the other indicators were significantly higher than those in control group, the difference between two groups was statistically significant (P 〈 0.05). Conclusion Comprehensive rehabilitation training can improve the motor function, pulmonary function, improve the quality of life and improve the activities of daily living in patients with stable chronic obstructive pulmonary disease.
