摘 要:目的从骨骼肌损伤标志酶的活性改变入手,探讨针刺对脑梗死后骨骼肌运动功能的改善作用,以进一步揭示其作用机制。方法健康雄性SD大鼠100只,体重(192±9)g,随机分为头针组、体针组、头针+体针组,模型组和空白组。每组20只。采用线栓法制作大鼠大脑中动脉局灶性脑梗死模型,并应用Zea-Longa的5级评分标准对造模大鼠进行筛选。每组分别于针刺治疗后即刻、24 h、48 h股动脉取血离心,检测ATP酶、CK、LDH活性。结果 (1)模型组ATP酶活性在0h明显低于空白组(P<0.001),并与24h至48h之间回升;与模型组相比,头针加体针治疗组在0h(P<0.001),24h(P<0.01),48(P<0.05)小时差异均具有统计学意义,体针组在24h有显著性差异(P<0.001),增加ATP酶活性的作用达到峰值;头针组在0h差异具有统计学意义(P<0.05)。(2)模型组CK活性在48h明显高于空白组(P<0.001);与模型组相比,三个针刺组中CK含量有明显变化,头针加体针治疗组在0h(P<0.05),24h(P<0.05),48h(P<0.01)差异均具有可比性;体针组在0h,48h有显著性差异(P<0.01);头针组在24h有显著性差异(P<0.05)(3)模型组CK活性在0h、12h、48h均明显高于空白组(P<0.001);与模型组比较,三个针刺组中LDH含量均有明显变化,头针加体针治疗组在0h(P<0.01)、24h(P<0.05)均具有可比性,体针组在48h有显著性差异(P<0.01),头针组在0h有显著性差异(P<0.01)。(4)头针+体针组与体针组在改变ATP、CK、LDH含量的变化方面无显著差异(P>0.05)。结论针刺可以增加ATP酶活性,降低血清CK及LDH活性,提高骨骼肌有氧酵解,增加肌细胞膜通透性,减轻骨骼肌损伤。Objective: To study the effect of acupuncture on skeletal muscle motor function after cerebral infarction, and to reveal its mechanism from the changes of skeletal muscle injury marker enzyme activity. Methods : 100 healthy male SD rats weighing 192±9 g were randomly divided into a scalp acupuncture group, a body acupuncture group, a scalp + body acupuncture group, a model group and a blank group, 20 rats per group. The suture method was used to establish rat models of focal cerebral infarction and Zea - Longa's five - lev- el scoring standard was used to screen the model rats. The activity of ATPase, CK and LDH of each group were measured by eentrifugation at 24 h and 48 h after acupuncture treatment. Results : 1. The activity of ATPase in the model group was significantly lower than that in the blank group (P 〈 0. 001 ) at Oh, and recovered from 24h to 48h. Compared with the model group, the scalp acupuncture group showed sig- nificant differences at Oh (P 〈 O. 001 ), 24h( P 〈 0.01 ), and at 48h (P 〈 0.05 ). The body acupuncture showed the significant difference at 24h( P 〈 O. 001 ) and the effect of increasing activity of ATPase reached the peak. The scalp acupuncture group showed the difference was statistically significant. 2. CK activity in the model group was significantly higher than that in blank group at 48h (P 〈 0.001 ). Compared with model group, CK contents in three acupuncture groups significantly changed at Oh ( P 〈 0.05 ), 24h ( P 〈 0.05 ) and 48h ( P 〈 0. 01 ). The differences were comparable. The body acupuncture group had significant difference at Oh and 48h ( P 〈 0.01 ). The scalp acu- puncture had significant difference at 24h ( P 〈 O. 05 ). 3. CK activity in the model group was significantly higher than that in the control group (P 〈 0.001 ) , at Oh, 12h and 48h.
