摘　　要：目的分析先天性心脏病术后施行早期静脉营养的低体质量患儿并发呼吸道感染危险因素,为提高医院感染控制工作提供依据。方法选取2014年2月-2016年8月行早期静脉营养支持治疗的低体质量先天性心脏病患儿106例为研究对象,对其术后呼吸道感染情况及相关因素进行观察和分析。结果 106例纳入患儿中共有16例发生呼吸道感染,感染率为15.1%;感染与非感染患儿在气管插管方面的差异有统计学意义(P<0.05);感染与非感染患儿在手术时间、延迟关胸、术后呼吸机使用时间等方面的差异均有统计学意义(P<0.05);Logistic多元回归分析结果显示,患儿术后呼吸道感染的发生与气管插管、延迟关胸、术后呼吸机使用时间具有相关性(P<0.05)。结论施行早期静脉营养治疗的先天性心脏病患儿术后呼吸道感染与多种因素有关,临床医务人员要给予高度关注和严格掌控,以达到预防术后感染、确保手术效果、改善患儿预后的目的。OBJECTIVE To explore the risk factors for complicated respiratory tract infection in low body mass children with congenital heart disease receiving postoperative early parenteral nutrition so as to provide guidance for control of nosocomial infections.METHODS A total of 106 low body mass children with congenital heart disease who received early parenteral nutrition support therapy from Feb 2014 to Aug 2016 were recruited as the study objects.The prevalence rate of postoperative respiratory tract infection was monitored.RESULTS Of 106 enrolled children,16 had respiratory tract infection,with the infection rate of 15.1%.There was significant difference in the tracheal intubation between the children with infection and the children without infection（P〈0.05）.There was significant difference in the operation duration,delayed sternal closure,or time of postoperative use of ventilator between the children with infection and the children without infection（P〈0.05）.The result of multivariate logistic regression analysis indicated that the incidence of postoperative respiratory tract infection in the children was associated with the tracheal intubation,delayed sternal closure,and time of postoperative use of ventilator（P〈0.05）.CONCLUSIONThe postoperative respiratory tract infection in the children with congenital heart disease undergoing the early parenteral nutrition support therapy is associated with multiple factors.It is necessary for health care workers to attach great importance and strictly grasp the factors so as to prevent the postoperative infection,ensure the surgical effect,and improve the prognosis of the children.
