枕寰枢复合体创伤的影像学诊断

摘　　要：目的:探讨枕寰枢复合体(C_0-C_1-C_2)创伤的影像学诊断。方法:回顾性分析40例枕寰枢复合体创伤的X线、三维CT及MRI表现。结果:X线平片发现枕寰枢复合体骨折共15例,寰枢关节脱位7例,枢椎滑脱2例。三维CT发现枕寰枢复合体骨折28例,骨折合并寰枢关节脱位10例,寰枕关节半脱位5例,寰枢关节无骨折型脱位或半脱位8例。X线、CT检查阴性4例。MRI检查示28例枕寰枢骨折患者椎体有骨髓挫伤水肿表现,18例延髓或颈髓形态或信号异常,15例椎旁软组织及韧带损伤。结论:联合X线、三维CT及MRI诊断枕寰枢复合体创伤,可提高诊断准确性,为临床治疗提供依据。Objective：To investigate the imaging diagnosis of the occipital ring complex （C0-C1-C2） trauma. Methods：X-ray. MRI and CT data of 40 patients with occipital ring complex trauma were retrospectively analyzed. Results：X-ray found 15 cases of fracture,7 cases of ring pivot joint dislocation. Three-dimensional CT examination revealed 28 cases of fracture,fracture combined with atlantoaxiai dislocation in 10 cases,ring pillow subluxation 5 cases,the ring-type pivot joint without fracture dislocation or subluxation of the 8 cases. X-ray,CT scan were negative in 4 cases~ MRI examination found 28 cases of fracture patients with vertebral bone marrow contusion and edema,18 cases of medulla oblongata and spinal cord injury,15 cases with vertebral soft tissue and ligament injury. Conclusion：X-ray.CT combined with MRI has the value in the diagnosis of the trauma of the occipital ring complex,may improve diagnostic accuracy,providing valuable evidence for clinical treatment.
