类孟买型血清学特点及后续输血策略的研究

摘　　要：目的鉴定并分析类孟买型血清学特点,探讨输血策略。方法采用ABO血型正反定型、H抗原检测及红细胞吸收放散试验,采用PCR-SSP法进行FUT1基因测序,采用配合型输血方案进行类孟买型输血策略研究。结果 6例被检者均确认为类孟买型,分别为Bm^h4例、Am^h2例;FUT1基因测序547-552del AG 4例、682 AG 2例;例1、例2与10份B型悬浮红细胞、10份O型洗涤红细胞进行交叉配血试验,结果显示在室温有2份交叉配血不合,在37℃交叉配血全部相合。结论罕见类孟买型血清学鉴定研究,对后续患者的输血策略具有重要意义。Objective To identify and to analyze the serological charateristics of para - Bombay phenotype, and to discuss the strategy of blood transfusion. Methods Using forward and reverse ABO blood typing, H antigen testing and erythrocytes abssorption - radiatieon testing, to sequence the gene of FUT1 by PCR - SSP method, and cooperative blood transfusion was used to study the blood transfusion strategy. Results 6 cases were identified as para - Bombay phenotype, including 4 cases of Bm^h and 2 cases of Am^h. There are 4 cases of 547 -552delAG and 2 cases of 682 AG in FUT1 gene sequencing; case 1 and case 2 had blood cross matching with 10 suspenged erythrocytes of type B and 10 washed erythrocytesof type O. The result showed that 2 blood cross - matching test was incompatible in room temperature, but all blood cross - matching test was compatible in 37℃. Conclusion The serological study of rare para - Bombay phenotype has important significance to blood transfusion strategy in subsequent patients.
