摘 要:目的探讨纤溶酶溶解血凝块的最佳时间及浓度。方法采集日本白兔股动脉血,应用模拟脑出血微创穿刺体外实验模型,采用5×5拉丁方设计,分析纤溶酶浓度(25 U/ml、50 U/ml、100 U/ml、200 U/ml及300 U/ml)、用药时机(抽血后2、4、6、8、16 h)和溶凝时间(给予纤溶酶后2、4、6、8、10 h)对体外血肿溶凝的时效和量效的影响。抽取兔股动脉血5 ml制备血凝块,实验管加相应浓度的纤溶酶0.2 ml,对照管加0.2 ml生理盐水,在各个时间点测定血凝块质量。结果兔动脉血实验数据结果显示,25 U/ml组(=8.496 mg)的溶凝质量明显低于50 U/ml组(=28.770 mg)、100 U/ml组(=38.904 mg)、200 U/ml组(=34.080 mg)、300 U/ml组(=28.324 mg),组间比较,差异有统计学意义[(B、C、D、E)>LSR_(0.05),P<0.05],而后4组间比较,差异均无统计学意义(P>0.05)。用药时机和溶凝时间对溶凝质量的影响,组间比较,差异均无统计学意义(F=0.62,P>0.05;F=1.44,P>0.05)。结论纤溶酶溶凝的最佳有效浓度为50 U/ml,延迟16 h用药不影响溶凝效果,溶凝2 h足以发挥作用。Objective To explore the optimal time and concentration for clot lysis with plasmin. Methods Blood samples were collected from the femoral artery of Japanese Rabbit. Using 5 × 5 latin square design,concentration of plasmin solution( 25 U/ml,50 U/ml,100 U/ml,200 U/ml and 300 U/ml),injected into the central of the blood clot after blood samples which were incubated for 2 hours,4 hours,6 hours,8 hours and 10 hours),the quality of the residual clots were measured at 2 hours,4 hours,6 hours,8 hours and 10 hours after the plasmin. The blood clots were collected from femoral artery of rabbits in 5 ml,the dose of 0. 2 ml concentration of plasmin solution in experimental tube,and 0. 2 ml saline was add in the control tube,the quality of blood clots were measured at every time point. Results The experimental data results of arterial blood showed that plasmin quality in 25 U/ml group( X= 8. 496mg)was significantly lower than that in 50 U/ml group( X= 28. 770 mg),100 U/ml group( X= 38. 904 mg),200 U/ml group( X=34. 080 mg),300 U/ml group( X= 28. 324 mg),there were statistically significant differences between the groups [X( B,C,D,E) LSR0.05,P〈0. 05],while there was no statistically significant difference among the latter 4 groups( P〉0. 05). There was no statistically significant difference between plasmin quality and time of medicine administration and plasmin( F = 0. 62,P〉0. 05; F = 1. 44,P〉0. 05). Conclusion In this experiment,the results suggest that optimal concentration of plasmin required clotted blood would be 50 U/ml,and delaying therapy of up to 16 hours does not significantly compromise its efficacy and 2 hours is quite sufficient for receiving good lytic efficacy with plasmin treatment.
