尿微量蛋白和Ⅳ型胶原检测对早期2型糖尿病肾病的诊断价值

摘  要:目的探讨尿微量蛋白、Ⅳ型胶原检测对早期2型糖尿病肾病的诊断价值。方法将85例2型糖尿病肾病患者设为观察组,65名健康志愿者设为对照组。采集相关标本检测分析两组尿微量蛋白、Ⅳ型胶原、尿白蛋白、糖化血红蛋白、肌酐水平,并对尿微量蛋白和Ⅳ型胶原水平与尿白蛋白排泄率、糖化血红蛋白、肌酐水平进行相关性分析。结果观察组尿微量蛋白、Ⅳ型胶原水平和阳性检出率及尿白蛋白排泄率、糖化血红蛋白、肌酐水平均显著高于对照组(P〈0.01);观察组尿微量蛋白水平与Ⅳ型胶原水平呈显著正相关(P〈0.01);尿微量蛋白、Ⅳ型胶原水平与尿白蛋白排泄率、糖化血红蛋白、肌酐水平均呈显著正相关(P〈0.01)。结论2型糖尿病肾病患者尿微量蛋白、Ⅳ型胶原水平明显升高,早期检测对2型糖尿病肾病的早期诊断具有重要意义。Objective To explore the Diagnostic value of microalbuminuria (MA) and type IV collagen (IV-C) detection to early type 2 diabetic nephropathy (T2DN). Methods Eighty-five T2DN patients were assigned to observation group and 65 healthy volunteers to control group. Relevant samples were collected to detect MA,IV-C, urinary albumin, glycosylated hemoglobin (HbAIC) and creatinine (Cr) levels in both groups and the correlation between MA and IV-C levels and urinary albumin excretion rate (UAER), HbA1C and Cr levels analyzed. Results MA and IV-C levels, positive detection rate as well as UAER, HbA1C and Cr levels were all significantly higher in observation than in control group (P〈0.01) ; MA and IV-C levels were positively related to UAER, HbA1C and Cr levels significantly (P〈0.01). Conclusion MA and IV-C levels of T2DN patients heighten notably and early detection has crucial significance for early diagnosis of T2DN.
