右美托咪定静脉泵注联合七氟烷吸入诱导在宫腔镜手术麻醉中的应用

摘　　要：目的:研究右美托咪定静脉泵注联合七氟烷吸入诱导在宫腔镜手术麻醉中的应用效果。方法:以笔者所在医院2015年5-12月宫腔镜手术麻醉患者90例为对象进行分组。Q组单纯给予七氟烷吸入诱导,Q+Y组则给予右美托咪定静脉泵注联合七氟烷吸入诱导。比较两组患者宫腔镜手术麻醉呼吸抑制和体动发生率;麻醉前和麻醉后患者心率、血氧饱和度、平均动脉压的差异,并比较两组患者诱导时置入喉罩的时间、复苏时间。结果:Q+Y组宫腔镜手术麻醉呼吸抑制和体动发生率低于Q组,差异有统计学意义(P<0.05);麻醉前两组心率、血氧饱和度、平均动脉压比较差异无统计学意义(P>0.05);麻醉后Q+Y组相较于Q组心率、血氧饱和度、平均动脉压波动幅度更低,差异有统计学意义(P<0.05);Q+Y组相较于Q组诱导时置入喉罩所需时间更短,复苏时间更短,差异有统计学意义(P<0.05)。结论:右美托咪定静脉泵注联合七氟烷吸入诱导在宫腔镜手术麻醉中的应用效果确切,可在保留自主呼吸情况下顺利置入喉罩,缩短喉罩置入时间,减少七氟醚用量,对循环系统影响小,具有良好的麻醉效果,且可加速术后复苏,值得推广。Objective： To investigate the effect of Dexmedetomidine intravenous pumping combined with Sevoflurane inhalation induction in the application of hysteroscopic surgery anesthesia.Method： In the author＇s hospital from May 2015 to December 2015, 90 cases of anesthesia patients were divided into two groups.The Q group was given Sevoflurane inhalation induction, Q＋Y group was given Dexmedetomidine intravenous pumping.Anesthetic respiratory depression and body movement incidence of two groups were compared.Before and after anesthesia heart rate, oxygen saturation, the mean arterial pressure difference, the LMA insertion time, and the recovery time of two groups were compared.Result： The anesthetic respiratory depression and body movement incidence of Q＋Y group was lower than Q group, the differences were statistically significant（P〈0.05）.There were no statistically significant differences among the heart rate, blood oxygen saturation, and the mean arterial pressure before the anesthesia（P〉0.05）.After anesthesia, the heart rate, oxygen saturation, and mean arterial pressure in Q＋Y group was lower than Q group, the differences were statistically significant（P〈0.05）.The LMA insertion time and the recovery time of Q＋Y group was shorter than the Q group, the differences were statistically significant（P〈0.05）.Conclusion： The application of Dexmedetomidine intravenous pumping combined with Sevoflurane inhalation induction in the application of hysteroscopic surgery anesthesia is exact, under the condition of retain spontaneous smoothly place LMA, shorten the LMA placement time, reduce the dosage of seven isoflurane, little effect on the circulatory system, has good anesthetic effect, and can accelerate the postoperative recovery, is worthy of promotion.
