摘 要:目的 探讨2型糖尿病(T2DM)患者颈动脉内膜中层厚度(cIMT)与轻度认知功能障碍(MCI)的关系.方法 收集124例T2DM患者的一般资料,检测其常规实验室指标,根据蒙特利尔认知评估量表(MoCA)评分将其分为正常认知功能组(NC组)51例和MCI组73例.通过颈动脉超声多普勒检测两组患者的颈总动脉、颈动脉窦、颈内动脉IMT,并比较分析其与MoCA评分的相关性.结果 NC组颈总动脉IMT低于MCI组(P〈0.05),两组患者颈动脉窦、颈内动脉IMT间比较差异无统计学意义(P〉0.05).颈总动脉IMT增厚(IMT≥1mm)组MoCA评分明显低于颈总动脉IMT正常组(IMT〈1mm)[分别为22(19,25)分和25(22,27)分,P=0.006].多元线性回归结果显示,MoCA评分仅与颈总动脉IMT(B=-0.133,P=0.035)及年龄(B=-0.008,P=0.002)呈负相关.结论 T2DM患者颈总动脉IMT与认知功能之间呈负相关,颈总动脉IMT越厚、年龄越大,认知功能越差.Objective To explore the relationship between the carotid artery intima-media thickness(cIMT) and mild cognitive impairment(MCI) in type 2 diabetes mellitus(T2DM) patients.Methods The general data of 124 patients with T2DM were collected and routine biochemical indexes were measured.Patients were assigned to normal cognitive function group(NC group)and MCI group according to Montreal cognitive assessment(MoCA)scores.IMT was measured in common carotid artery,bulbus caroticus and internal carotid artery.IMT was compared between two groups and the correlation between IMT and MoCA scores was analyzed.Results Common carotid artery IMT of NC group was lower than that of MCI group(P〈0.05).Differences of IMT in internal carotid sinus and internal carotid were unsignificant(P〉0.05).The MoCA scores in thickened common carotid artery IMT(IMT≥1mm) group were significantly lower than those in normal common carotid artery IMT(IMT〈1mm) group[22(19,25)and 25(22,27)respectively,P=0.006].Multiple linear regression showed that MoCA score was negatively correlated with common carotid artery IMT(B=-0.133,P=0.035) and age(B=-0.008,P=0.002).Conclusion Independent for other factors,common carotid artery IMT is negatively related with cognitive function in type 2 diabetes patients.Cognitive function becomes worse along with the increase of age and thickness of common carotid artery IMT.
