微泵输注瑞芬太尼复合丙泊酚在小儿腹腔镜疝环结扎术中的应用

摘　　要：目的探讨微泵输注瑞芬太尼复合丙泊酚在小儿腹腔镜疝环结扎术中的应用效果。方法选取我院2015年7月至2016年7月收治的80例行腹腔镜疝环结扎术的斜疝患儿,随机分为观察组与对照组,各40例。两组均予以舒芬太尼、丙泊酚、苯磺酸顺阿曲库铵进行全麻诱导,在此基础上,观察组采用微泵输注瑞芬太尼复合丙泊酚维持,对照组采用七氟醚吸入、丙泊酚泵注维持。比较两组患儿的手术指标、血流动力学变化,以及拔管后苏醒期躁动、呼吸困难、低血氧、恶心呕吐情况。结果观察组的拔管时间和苏醒时间均显著短于对照组,血流动力学稳定性优于对照组,拔管后呼吸困难、苏醒期躁动和低血氧发生率均明显低于对照组,组间比较差异均有统计学意义(P均<0.05)。结论在小儿腹腔镜疝环结扎术中应用微泵输注瑞芬太尼、丙泊酚维持麻醉,可稳定血流动力学,促进患儿苏醒,可控且安全,值得推广。Objective To explore the application of micropump infusion of remifentanil combined with propofol in pediatric laparoscopic hernia ring ligation. Methods 80 cases of children with oblique hernia that received laparoscopic hernia ring ligation in our hospital from July 2015 to July 2016 were selected and randomly divided into observation group and control group, with 40 cases in each group. Both groups were given general anesthesia induction of sufentanil, propofol and cisatracurium besylate. On this basis, the observation groups was maintained by micropump infusion ofremifentanil combined with propofol, and the control group was maintained by sevoflurane inhalation and propofol infusion. The operation indicators, hemodynamic changes and the condition of agitation during recovery period, dyspnea, hypoxemia, nausea and vomiting after extubation were compared between two groups. Results The extubation time and recovery time of observation group were significantly shorter than those of control group, the hemodynamics of observation group was more stable than that of control group, and the incidences of dyspnea, agitation during recovery period and hypoxemia of observation group were significantly lower than those of control group. The above differences between two groups were statistically significant （all P 〈0.05）. Conclusions Micropump infusion of remifentanil combined with propofol in the application of pediatric laparoscopic hernia ring ligation maintenance can stabilize the hemodynamics and promote children＇s recovery, which is controllable and safe, and worthy of promotion.
