孙志教授“泻胃补脾法”针刺治疗2型糖尿病经验浅析

摘　　要：[目的]浅析孙志教授"泻胃补脾法"针刺治疗2型糖尿病的临床经验。[方法]从2型糖尿病病机新认识、刺法理论研究、临床实践等方面,总结孙志教授"泻胃补脾针法"的腧穴配伍、针刺手法,体会其治疗特色,并举案例佐证。[结果]2型糖尿病属于中医"消渴病"范畴,但其病机与消渴不尽相同,孙志教授根据多年临床观察发现2型糖尿病发病多见肥胖、消谷善饥并伴见倦怠乏力,概括其病机为"胃强脾弱",虚则补之,实则泻之,使用补泻手法针刺,调节脾胃功能,泻法针刺中脘、足三里、内庭抑胃控制食欲,补法针刺太白、三阴交、脾俞增强脾的运化,以达泻胃补脾之用,在临床上治疗2型糖尿病取得良好疗效。[结论]孙志教授"泻胃补脾法"为针刺治疗糖尿病提供了新的思路,值得进一步探索及推广应用。[Objective] Analyze the academic thought of＂purge stomach and tonify spleen＂treatment used by Professor SUN Zhi in acupuncture on type-2 diabetes mellitus during his clinical experience.[Method] According to new recognition of type-2 diabetes mellitus pathogenesis, study of acupuncture theory,clinical manipulation, conclude acupuncture point choices and needling methods of＂purge stomach and tonify spleen＂treatment, studying its characteristics.[Result] Type-2 diabetes mellitus belonging to ＂Xiaoke＂ category which features excessive discharge of urine and persistent thirst in Chinese medicine, but their pathogenesis is not the same, Professor SUN Zhi based on years of clinical observation found that patients with type-2 diabetes mellitus commonly manifested obese, polyorexia and accompanied by fatigue, summarized as ＂strong stomach and weak spleen＂. Tonify the deficiency and purge sthenia, using acupuncture manipulation, regulating spleen and stomach function, by reducing method of acupuncture Zhongwan, Zusanli and Neiting to inhibt stomach and control appetite, reinforcing method of acupuncture Sanyinjiao, Taibai, Pishu to enhance the transport of the spleen, meaning purging stomach and tonifying spleen. This treatment of type-2 diabetes mellitus in clinic achieved good results. [Conclusion]＂Purge stomach and tonify spleen＂treatment offers a new way of acupuncture to treat diabetes, deserving further exploration, popularization and application.
