鼻内镜术后术腔纳吸绵的处理方式研究

摘　　要：目的探讨鼻内镜术后鼻腔填塞物纳吸绵的取出时间与处理方式。方法将120例双侧慢性鼻-鼻窦炎伴或不伴鼻息肉患者随机分成2组,行鼻内镜手术后均使用纳吸绵填塞。A组于术后第3天清理填塞物,并开始行鼻腔冲洗;B组不取出填塞物,于术后第3天行鼻腔冲洗。术后2周清理纳吸绵降解残余物。对比2组患者术后短期舒适度及术创情况。结果术后第5天,鼻部舒适度评估:A组优于B组,差异有统计学意义(P<0.01);术后2周,鼻部舒适度评估:2组差异无统计学意义(P>0.05);术后4周,A组术腔黏膜恢复情况评分不及B组,差异有统计学意义(P<0.01)。结论鼻内镜术后使用纳吸绵填塞,待其自然降解利于鼻腔黏膜术后恢复,减少鼻腔术创粘连;但会增加患者不适感,应根据具体情况选择处理方式。Objective To investigate the take-ff time and the treatment method of NasoPore as nasal packing material after nasal endoscopic surgery. Methods One hundred and twenty patients with bilateral chronic nasal sinusitis with or without nasal polyps were randomly divided into two groups. All patients were packed with NasoPore after endoscopic sinus surgery. In group A, the packing material was cleaned up three days after the surgery, and the nasal cavity was washed. In group B, the stuffing was not removed, and the nasal cavity was also washed on the third day after the surgery. Two weeks after the operation, residue degradation of NasoPore was cleaned. The comfort degree and the nasal cavtity situation of the two groups was compared postoperatively. Results Five days after the surgery, the nasal comfort assessment of group A was better than group B, and the difference was statistically significant （ P 〈 0.01 ） ; two weeks after the operation, no significant difference between two groups was observed in the nasal comfort assessment（ P 〉 0.05 ） ; four weeks after the operation, mucosa recovery score in group A was less than in group B, and the difference was statistically significant （ P 〈 0. 01 ）. Conclusions Natural degradation of NasoPore is beneficial for nasal mucosa recovery after endoscopic sinus surgery, and can reduce the adhesion of nasal cavity, but it will increase the patients＇ disconffort.
