吻合器痔上黏膜环形切除术和传统痔切除术治疗混合痔的效果对比

摘　　要：目的探讨传统痔切除术(MMH)和吻合器痔上黏膜环形切除术(PPH)治疗混合痔的效果差异。方法 120例混合痔患者作为研究对象,根据手术方式不同分为对照组(58例)与实验组(62例)。对照组患者采用MMH术式,实验组患者采用PPH术式。观察两组患者手术时间、术后疼痛时间、住院时间、术后并发症及复发情况的差异。结果两组患者混合痔手术治疗均有效;实验组的手术时间(23.5±5.1)min、术后疼痛时间(2.3±0.9)d及住院时间(4.4±1.1)d均明显短于对照组(32.6±6.2)min、(4.5±1.1)d、(8.6±1.3)d,差异均有统计学意义(t=-8.80、-12.02、-19.15,P<0.05)。实验组患者术后并发症发生率14.5%低于对照组29.3%,差异具有统计学意义(χ~2=3.86,P<0.05);院外随访6个月,两组患者术后复发率比较差异无统计学意义(χ~2=0.28,P>0.05)。结论相较于MMH,采用PPH术式治疗混合痔效果更佳,术后恢复快,并发症少,值得临床推广。Objective To explore the difference on effect of Milligan-Morgan hemorrhoidectomy（MMH） and procedure for prolapse and hemorrhoids（PPH） in the treatment of mixed hemorrhoid. Methods A total of 120 mixed hemorrhoid as study subjects were divided by different ways of operation into control group（58 cases） and experimental group（62 cases）. The control group received MMH operation, and the experimental group received PPH operation. Observation were made on the difference of operation time, postoperative pain time, hospital stay time, postoperative complications and recurrence in two groups. Results Both two groups had effective mixed hemorrhoid operations in patients. The experimental group had operation time as（23.5±5.1） min, postoperative pain time as（2.3±0.9） d and hospital stay time as（4.4±1.1） d, which were all better than（32.6±6.2） min,（4.5±1.1） d and（8.6±1.3） d in the control group. Their difference had statistical significance（t=-8.80,-12.02,-19.15, P〈0.05）. The experimental group had incidence of postoperative complications as 14.5%, which was lower than 29.3% in the control group, and the difference had statistical significance（χ^2=3.86, P〈0.05）. After 6 months of outside-hospital follow-up, both groups had no statistically significant difference in postoperative recurrence rate（χ^2=0.28, P〈0.05）. Conclusion Compared with MMH, PPH operation provides better effect for the treatment of mixed hemorrhoids with quicker recovery and fewer complications, and it is worthy of clinical promotion.
