摘  要:目的评估房颤和急性缺血性脑卒中的严重程度和早期死亡的相关性。方法将纳入的86例急性缺血性脑卒中患者分成房颤组(40例)和非房颤组(46例),按照美国国立卫生研究院健康卒中评分(NIHSS)标准,比较两组患者的分值,平均住院日,早期死亡率,用多因素Logistic回归分析房颤和急性缺血性脑卒中的严重程度和早期死亡之间的关系。结果房颤组的NIHSS评分、平均住院日和早期死亡率明显高于非房颤组(P<0.05)。多因素Logistic回归分析发现,男性、高龄、房颤是严重急性缺血性脑卒中的独立危险因素(P<0.05)。男性、高龄、糖尿病、房颤和高NIHSS分值是早期死亡相关的独立危险因素(P<0.05)。结论房颤是急性缺血性卒中患者严重中风和早期死亡的相关危险因素,评估和干预房颤高危人群对预防急性缺血性卒中极为重要。Objective To assess the relation between atrial fibrillation( AF) and the severity and early death in patients with acute ischaemic stroke. Methods 86 patients with acute ischemic stroke were divided into AF group( n = 40)and Non-AF group( n = 46).The National Institutes of Health Stroke Scale( NIHSS) scores,average length of stay in hospitals and early death were compared. Multivariate Logistic regression analysis was used to evaluate the association of stroke severity and early death with AF. Results The NIHSS scores,average length of stay in hospitals and early death rate in AF group were significantly higher than that in Non-AF group( P〈0.05).Multivariate Logistic regression analysis identified that male,older age,AF as the independent risk factors were associated with severe stroke( P〈0.05).Moreover,male,older age,diabetes,AF and high NIHSS scores were the independent risk factors associated with early death( P〈 0.05). Conclusion AF is an independent risk factor for severe stroke and early death in acute ischemic stroke.Careful evaluation and treatment for high-risk AF patients is beneficial for preventing acute ischemic stroke.
