电子胃镜在小儿上消化道疾病中的诊断探讨

摘　　要：目的探讨电子胃镜在小儿上消化道疾病中的诊断价值。方法选取2013年6月—2017年3月于本院消化科就诊行胃镜检查的508例儿童作为研究对象。患儿均先行电子胃镜检查,再行钡餐检查,分别统计本组患儿电子胃镜和钡餐透视检查诊断结果,分类统计各类上消化道疾病检查情况及总检出率。结果电子胃镜检查上消化道疾病检出率94.49%,高于钡餐检查率(74.02%),差异有统计学意义,P<0.05。电子胃镜检查和钡餐透视检查均显示慢性浅表性胃炎检出率最高(50.79%,40.35%),十二指肠溃疡次之(22.44%,18.11%)。508例上消化道疾病患儿中,幽门螺杆菌阳性381例(75.00%),阴性127例(25.00%),幽门螺杆菌阳性者居多。结论电子胃镜可提高小儿上消化道疾病中检出率,有助于降低上消化道疾病漏诊率,临床诊断价值较高。Objective To investigate the diagnostic value of electronic gastroscope in children with upper digestive tract diseases. Methods 508 cases of children who underwent gastroscopy in the department of gastroenterology in our hospital from June 2013 to March 2017 were selected as subjects. All patients were examined by electronic gastroscopy and barium meal examination. The results of electronic gastroscopy and barium meal X-ray examination were analyzed respectively. Results The detection rate of upper gastrointestinal diseases was 94.49%, which was significantly higher than that of barium meal examination （74.02%）,and the difference was significant （P〈0.05）. The results of electronic gastroscopy and barium meal examination showed that the detection rate of chronic superficial gastritis was the highest （50.79%, 40.35%）, followed by duodenal ulcer （22.44%, 18.11%）. 508 cases of children with upper digestive tract disease, Helicobacter pylori positive in 381 cases （75.00%）, negative in 127 cases （25.00%）, helicobacter pylori positive majority. Conclusion Electronic gastroscope can improve the detection rate of upper digestive tract diseases in children, help to reduce the rate of missed diagnosis of upper digestive tract disease, the clinical diagnostic value is higher.
