摘  要:目的根据机械通气致新生儿医院感染发生特点采取针对性防控措施,降低机械通气新生儿医院感染发生率。方法对2011-2014年四川省妇幼保健院NICU机械通气的382例新生儿按不同出生体重、住院天数以及插管天数进行预后差异分析。结果发生医院感染的49例新生儿中,肺部感染主要发生在出生体重>1 500 g、住院天数>7 d的患儿,血液系统感染主要以体重≤1 500 g及住院时间≤7 d的患儿;插管天数与感染部位无统计学意义(P=0.126)。结论采用机械通气患儿医院感染部位主要为肺部及血液系统。住院早期体重越低,血液系统感染几率越高,与插管天数无相关;体重相对较高患儿随着住院时间的增加,肺部感染越高;机械通气第一周内发生医院感染的机率较高,应根据不同的具体情况采取相应的防控措施,减少医院感染的发生。ObjectiveTo investigate the infection features of nosocomial infection in neonate with ventilation, and to adopt individual prevention and control measures to decrease the incidence of nosocomial infection in neo- nates, nethodsFrom 2011 to 2014, 382 neonatal cases with ventilation in NICU were analyzed to explore the differences on birth weight, length of hospitalization stay and ventilation days. ResultsThere were 49 cases had nosocomial infection. Pulmonary infection mainly occurred in the cases of birth weight 〉 1 500 g and length of hospitalization stay 〉 7 days. Blood system infection mainly occurred in the cases of birth weight ≤1 500 g and length of hospitalization stay≤7 days. And there was no significant difference between the ventilation days and positions (P = 0. 126). ConclusionNosocomial infection sites of neonates with ventilation were mainly lung and blood. Neonates with the lower birth weight associated with higher incidence of blood infection nosocomial with- out ventilation days. The cases of higher birth weight with the increasing hospitalization stay were associated with higher rate of pulmonary infection. The incidence of nosocomial infection was higher in the first week of hospitalization. We should adopt different prevention and control measures according to different situations of the neonates, to decrease the incidence of nosocomial infection.
